重组人粒细胞巨噬细胞集落刺激因子对儿童浅度烧伤创面愈合的影响和安全性探讨

【摘要】:目的:探讨外用重组人粒细胞巨噬细胞集落刺激因子(Granulocyte/Macrophage Colony-stimulating Factor,rhGM-CSF)及外用重组牛碱性成纤维细胞生长因子(Recombinant Bovine Basic FibroblastGrowth Factor,rb-bFGF)治疗儿童浅Ⅱ度烧伤创面的有效性及安全性。 方法:将2012年6月-2012年11月住院的浅Ⅱ度烧伤患儿45例按照抽签法随机分为3组,治疗组分别为rhGM-CSF组20例、rb-bFGF组20例,对照组生理盐水组5例;rhGM-CSF组予0.5μg/cm2直接涂在创面上,,rb-bFGF组应用0.03ml/cm2直接均匀喷于创面,生理盐水组应用0.03ml/cm2直接均匀喷于创面,比较治疗组与对照组创面愈合时间,同时监测血常规、尿常规及肝肾功等相关指标,检测创面细菌培养。 结果:治疗组rhGM-CSF创面愈合时间(8.619±0.944)d,rb-bFGF组(10.766±1.369)d,两组较生理盐水对照组(13.850±0.398)d明显缩短,差异有统计学意义(P0.05);rhGM-CSF创面愈合时间(8.619±0.944)d较rb-bFGF组(10.766±1.369)d缩短,差异具有统计学意义。用药期间患儿血常规及肝肾功等相关指标未见明显异常,与药物使用前相比,P>0.05,差异无统计学意义,且在治疗过程中,患儿无明显不良反应。 结论:rhGM-CSF及rb-bFGF治疗浅Ⅱ度烧伤与生理盐水相比较能有效缩短愈合时间、加快愈合速度;rhGM-CSF在加快创面愈合速度上较rb-bFGF更为明显。在治疗过程中患儿无明显不良反应,在儿童浅度烧伤创面的治疗应用中具有安全性。
