化疗对急性淋巴细胞白血病患儿骨代谢近期影响的初步探讨

【摘要】:目的:探讨化疗对急性淋巴细胞白血病(acute lymphoblastic leukemia, ALL)患儿骨代谢的近期影响,比较不同疗程对骨代谢改变的差异。 方法:采取ALL患儿的116例血清样本,利用电化学发光法检测骨代谢指标:血清Ⅰ型胶原氨基端前肽(PINP)、Ⅰ型胶原交联羧基端肽(CTX)。治疗方案为2008-CCLG,时间点分别为:19例诱导缓解DOLP(DNR、VCR、L-Asp、Dex)疗程前、后,14例CAM(CTX、Ara-C、6-MP)第一疗程前、后,29例延迟强化DOLP(ADM、VCR、L-Asp、Dex)疗程*后(标危2例,中危13例,高危14例)、21例维持治疗(标、中危:6-MP+MTX/VD,高危:6-MP+MTX/CA/VD,)阶段(半年)。同期健康体检儿童18例设为对照组。(*注:中、高危组为延迟强化DOLPⅡ后) 结果:(1)诱导缓解DOLP疗程前、后、对照组的PINP值分别为(182.47±112.83)ng/ml、(250.16±316.65)ng/ml、(549.42±285.50)ng/ml,疗程前后PINP差别无统计学意义(P0.05),但与对照组比较均有显著性差异(P0.05); CTX值分别为(1.66±0.64)ng/ml、(1.95±1.08)ng/ml、(1.20±0.35)ng/ml,疗程前后CTX比较差异有显著性(P0.01),疗程后CTX疗程前CTX,且均大于对照组,差异有显著性(P0.05)。(2)CAM疗程前、后的PINP值分别为(617.49±430.83)ng/ml、(665.86±240.02)ng/ml,两组比较无统计学意义(P0.05)。疗程前、后PINP分别与对照组比较,均无显著性差异(P0.05);疗程前、后CTX分别为(1.90±0.72)ng/ml、(1.79±0.38)ng/ml,两组比较无统计学意义(P0.05)。分别与对照组比较,,差别均有显著性(P0.05)。(3)延迟强化DOLP疗程后标+中危组与高危组PINP分别为(357.57±219.37)ng/ml、(319.30±253.44)ng/ml,两组比较无显著差异(P0.05),但两组分别与对照组比较,均有显著性差异(P0.05);标+中危组与高危组CTX分别为(1.51±0.62)ng/ml、(1.63±0.31)ng/ml,两组比较无显著差异(P0.05),但两组分别与对照组比较,均有统计学意义(P0.05)。(4)维持期PINP、CTX分别为(631.89±215.54)ng/ml、(1.23±0.31)ng/ml,与对照组比较均无显著差异(P0.05)。 结论:(1)ALL患儿化疗前存在骨代谢异常,表现为骨合成抑制,骨破坏增强,考虑与白血病细胞浸润相关;(2)ALL患儿化疗期间存在骨代谢异常,与正常对照相比,化疗早期以骨破坏增强为主,骨合成抑制;至维持治疗期,骨合成、骨分解代谢基本平衡;(3)不同疗程对骨代谢影响不同。诱导缓解DOLP疗程促进骨破坏,CAM疗程对骨代谢无明显影响,化疗后期不同危险组患儿的骨代谢无显著差别。
