磁敏感加权成像在CT阴性的弥漫性轴索损伤中的应用

【摘要】:目的:探讨磁敏感加权成像(SWI)在CT阴性的弥漫性轴索损伤(Diffuse axonal injury,DAI)中的诊断价值。方法 :34例CT为阴性而临床可疑为DAI的患者在外伤后3 d内常规在3.0T磁共振行SWI和常规序列T1WI、T2WI、液体衰减翻转恢复序列(FLAIR)扫描,比较它们对DAI的检出率和检出病灶数目,评价SWI在诊断中的作用。结果:SWI可清楚显示27例DAI的颅内异常小出血灶,检出率达79.41%,常规T1WI、T2WI、FLAIR序列对DAI检出率分别为20.5%、26.47%和41.17%;SWI、T1WI、T2WI、FLAIR各序列检出微小出血量分别为261、39、61、89,SWI与T1WI、T2WI和FLAIR的差异有显著的统计学意义。结论:SWI对DAI出血性病灶的检出具有极高的敏感性,能够检出更多、更小病灶,且病灶显示更加清晰,在临床上可疑为DAI的患者应尽早行SWI序列扫描明确诊断。
[Abstract]:Objective: to investigate the application of magnetic sensitive weighted imaging (MRI) in diffuse axonal injury (axonal injury) in CT negative patients. Methods 34 patients with negative CT and suspected DAI underwent SWI and conventional T1WI at 3.0T within 3 days after trauma. T2WI, fluid attenuated flip recovery sequence (FLAIR) scanning, to compare their detection rate of DAI and the number of lesions detected. Results 27 cases of DAI showed small intracranial hemorrhage clearly, the detectable rate was 79.41%. Routine T1WI T2WI was used to evaluate the value of SWI in diagnosis. The positive rate of FLAIR sequence on DAI was 26.47% and 41.17%, respectively. The minimal blood loss detected by flair sequence of T _ 1WI _ I ~ T _ 2WI _ I _ T _ 1W _ I was 261T _ (39) ~ 61T _ (89) WI and T _ 1WI, respectively. The difference between T2WI and FLAIR was statistically significant. Conclusion the sensitivity of FLAIR to detecting hemorrhagic lesions of DAI is very high, and it can detect more and smaller lesions. The lesions were more clearly displayed, and the patients suspected to be DAI should be diagnosed by SWI sequence scan as soon as possible.
