弥散张量成像躯体化障碍脑微结构研究

【摘要】:目的:运用磁共振弥散张量成像定量指标测定躯体化障碍(SD)患者脑内超微结构的变化,以期对躯体化障碍的发病机制、临床研究提供有价值的依据。方法:采用3.0T磁共振对28例SD患者及28例正常对照进行DTI扫描,测量受试者左右侧额叶白质纤维、胼胝体、左右侧扣带束、左右上、下纵束感兴趣区内主要白质纤维束以及左右侧小脑齿状核的表观弥散系数(ADC)和各向异性分数(FA)值,并进行两组间比较,评估其在病例组和对照组之间的差异,分析两组间FA值有统计学差异的脑区的FA值与病程的相关性。结果:患者组胼胝体膝部FA值较对照组降低,左侧下纵束、左右侧扣带束水平部、左扣带束后部的ADC值较对照组明显升高,FA值较对照组明显降低(P0.05),有差异脑区的FA值与病程之间无统计学相关性。SD组右侧小脑齿状核FA值较左侧升高,二者之间差异有统计学意义,正常组左右侧小脑髓质和齿状核、正常和病例组间相应小脑髓质和齿状核ADC、FA值比较无统计学差异。结论:躯体化障碍患者优势半球扣带束、下纵束神经纤维完整性降低,存在超微结构异常,SD患者双侧小脑齿状核存在明显不对称性,可能是躯体化障碍患者病理生理基础之一,但是病程与神经纤维损伤程度未发现明显相关性。
[Abstract]:Objective: to determine the changes of brain ultrastructure in patients with somatization disorder (SDD) by using the quantitative index of magnetic resonance diffusion Zhang Liang imaging, in order to explore the pathogenesis of somatization disorder. Methods: DTI scanning was performed on 28 SD patients and 28 normal controls with 3.0T magnetic resonance imaging. The white matter fibers of left and right frontal lobe, corpus callosum, cingulate band of left and right were measured. The apparent diffusion coefficient (ADCC) and anisotropic fraction (FAA) of the main white matter fiber bundle and the left and right cerebellar dentate nuclei in the region of interest of the lower longitudinal bundle were compared and compared between the two groups to evaluate the difference between the case group and the control group. Results: the FA value of the genu of corpus callosum in the patients group was lower than that of the control group. The ADC value of the posterior part of the left cingulate band was significantly higher than that of the control group. The FA value of the right cerebellar dentate nucleus in SD group was higher than that of the left cerebellar dentate nucleus. There was no significant difference in ADCFA between the left and right cerebellum medulla and dentate nucleus in normal group, and the corresponding ADCFA value in cerebellar medulla and dentate nucleus between normal group and case group. Conclusion: the dominant hemispheric cingulate bundle is found in patients with somatization disorder. The nerve fiber integrity of the lower longitudinal bundle decreased and the bilateral dentate nucleus of the cerebellum showed obvious asymmetry in the patients with abnormal ultrastructure, which may be one of the pathophysiological bases of the patients with somatization disorder. However, no significant correlation was found between the course of disease and the degree of nerve fiber injury.
