不同偏侧性Rolandic癫痫的同步EEG-fMRI研究

【摘要】:目的:采用同步脑电图联合功能磁共振(EEG-fMRI)分析Rolandic癫痫(RE)患者脑区激活区域偏侧性与脑电图(EEG)偏侧性的关系。方法:根据病例选择标准,共纳入19例RE患儿。采用EEG-fMRI技术采集患儿脑功能数据,排除无发作间期痫样放电(IED)的和头动过大的病例,获得12例患儿的fMRI数据。采用广义线性模型(GLM)方法分析IED相关血氧水平依赖(BOLD)信号改变,计算脑区激活偏侧系数及EEG偏侧系数,并分析两者的相关性。结果:左侧及右侧IED为主的组分析显示BOLD信号激活主要位于同侧外侧裂-中央区,个体分析显示10例患儿外侧裂-中央区范围内BOLD信号显著激活。中央后回和中央沟盖区、中央前回脑区激活偏侧系数与EEG偏侧性显著相关(P0.01)。结论:fMRI上激活脑区和EEG信号偏侧性具有相关性,中央后回和中央沟盖区最适合作为描述RE偏侧性的目标脑区。
[Abstract]:Objective: to analyze the relationship between the laterality of the activated area and the hemiplegia of electroencephalogram (EEG) in patients with Rolandic epilepsy by synchronous EEG combined with functional magnetic resonance imaging (EEG-fMRI). Methods: according to the criteria of case selection, 19 children with RE were included. Brain function data were collected by EEG-fMRI technique to exclude the patients without interictal epileptiform discharges (IEDs) and large head movement. FMRI data were obtained from 12 children. The changes of IED related blood oxygen level dependent Bod signals were analyzed by using generalized linear model (GLM), and the activation hemivariate coefficients and the EEG hemicoefficients were calculated. Results: the BOLD signal activation was mainly located in the ipsilateral lateral fissure and central region in the left and right IED groups. Individual analysis showed that the BOLD signal was activated significantly in the lateral fissure and central area in 10 cases, the posterior central gyrus and the central sulcus cover area were significantly activated. There is a significant correlation between the activation hemiplegia coefficient of precentral gyrus and the hemiplegia of EEG. Conclusion there is a correlation between the activated brain area on EEG and the laterality of EEG signal, and the posterior central gyrus and the central sulcus tectum are the most suitable target areas for describing RE laterality.
