脊柱恶性血管源性肿瘤的影像表现及鉴别诊断

【摘要】:目的:探讨脊柱恶性血管源性肿瘤的影像表现。方法:对9例经病理确诊的脊柱恶性血管源性肿瘤的CT和MRI表现进行回顾性分析并总结其影像特点。结果:9例中2例为血管肉瘤,7例为上皮样血管内皮瘤;5例病变位于颈椎,3例位于胸椎,1例位于腰椎;4例为单中心、2例为多中心病变,3例累及相邻椎体。CT表现:9例均显示溶骨性骨破坏、周围骨皮质不完整,其中5例肿瘤内部可见残存骨嵴。MRI表现:肿瘤于T1WI呈低信号,于T2WI呈混杂信号(3例)、中等信号(2例)或高信号(4例);增强扫描示肿瘤呈明显强化4例,呈中等强化1例。结论:恶性血管源性肿瘤主要影像表现为单中心性或多中心溶骨性骨破坏、多位于颈胸椎,部分具有血管瘤的特点,容易导致脊柱压缩变形。
[Abstract]:Objective: to investigate the imaging features of spinal malignant vasogenic tumors. Methods: Ct and MRI findings of 9 cases of spinal malignant vasogenic tumors confirmed by pathology were retrospectively analyzed and their imaging features were summarized. 2 cases of angiosarcoma 7 cases were epithelioid angioendothelioma 5 cases were located in cervical vertebrae, 3 cases were located in thoracic vertebrae, 1 case was located in lumbar vertebra, 4 cases were located in single center, 2 cases were polycentric lesions, 3 cases involved adjacent vertebral bodies, 9 cases showed osteolytic bone destruction, 9 cases showed osteolytic bone destruction. The residual bone crest was found in 5 cases. MRI showed that the tumor showed low signal intensity on T1WI, mixed signal on T2WI in 3 cases, moderate signal intensity in 2 cases) or hyperintense lesions in 4 cases, enhanced obviously in 4 cases on contrast enhanced scan. Conclusion: the main imaging features of malignant angiogenic tumors are monocentric or multi-center osteolytic bone destruction, most of them are located at the cervical and thoracic vertebrae, and some of them have the characteristics of hemangioma, which can easily lead to compression and deformation of the spine.
