产前超声测量正常胎儿主动脉峡部收缩指数的临床价值

【摘要】:目的 探讨正常胎儿的主动脉峡部收缩指数范围,并对其同孕周之间的关系进行分析。方法 随机选取于漯河市第三人民医院2013年2月至2015年4月出生的251例正常胎儿,产妇孕周18~37周,对所有胎儿均进行频谱多普勒超声检查,对其主动脉峡部血流频谱图像进行分析,并对收缩末期最低点血流速度(NS)、收缩期峰值血流速度(PSV)进行测量,进行主动脉峡部收缩指数(ISI)计算,并对其同孕周之间的关系进行分析。结果 孕18~24周胎儿的ISI值位于0.19~0.21,随着胎儿孕周的增大,其NS逐渐降低,而PSV则呈现逐渐升高的趋势;在孕28周左右,胎儿的主动脉下部血流频谱可见明显收缩末期减速,此种趋势造成在孕30周附近ISI接近0,之后ISI的下降趋势持续增加,在妊娠末期胎儿的ISI平均值约为-0.33。结论 正常胎儿的主动脉峡部ISI、NS、PSV同孕周呈曲线相关性,通过对胎儿进行产前主动脉峡部收缩指数分析,可为脑血管畸形、心血管畸形等先天性畸形发育的诊断及评估提供可靠的依据。
[Abstract]:Objective to explore the range of aortic isthmus contraction index of normal fetuses and analyze the relationship between isthmus index and gestational age. Methods 251 normal fetuses born in Luohe third people's Hospital from February 2013 to April 2015 were randomly selected. At 18 to 37 weeks of gestation, all fetuses were examined by spectral Doppler ultrasound, and the spectral images of aortic isthmus blood flow were analyzed, and the lowest systolic blood flow velocity (NSV) and peak systolic blood flow velocity (PSV) were measured. The isthmus contraction index (ISI) of aorta was calculated and the relationship between isthmus and gestational weeks was analyzed. Results the ISI of fetuses at 1824 weeks of gestation was 0.19 卤0.21. With the increase of gestational weeks, NS decreased gradually, while PSV increased gradually. At 28 weeks of gestation, the blood flow spectrum of the lower aorta of the fetus showed a significant deceleration at the end of contraction, which resulted in the ISI approaching zero near the 30th week of gestation, and the decreasing trend of ISI continued to increase after 30 weeks of gestation. The average value of ISI in fetuses at the end of pregnancy is about -0.33. Conclusion the isometric isthmus of normal fetuses has a curve correlation with gestational weeks. By analyzing the contraction index of isthmus of aorta before delivery, it is suggested that the isthmus isthmus of normal fetuses is cerebrovascular malformation. The diagnosis and evaluation of congenital malformation such as cardiovascular malformation provide reliable basis.
