浸润性乳腺癌Ki-67表达与MRI形态学表现及ADC值间的相关性研究

【摘要】:目的探讨浸润性乳腺癌细胞增殖指数Ki-67表达与MRI形态学、表观扩散系数(ADC)之间的相关性。方法选取2013年9月至2015年12月间85例浸润性乳腺癌患者作为研究对象,所有患者术前均行常规MRI、扩散加权成像(DWI)及动态增强扫描,观察乳腺癌的MRI特征并与Ki-67表达情况进行对比分析。结果 85例浸润性乳腺癌Ki-67指数为5%~70%,其中高表达58例,低表达27例;无腋窝淋巴结转移59例Ki-67均值为(30.08±13.342)%,伴有淋巴结转移26例Ki-67均值为(40.19±12.999)%,两组Ki-67指数差异性显著(P=0.002,0.05);肿瘤伴有坏死29例Ki-67均值为(38.79±13.070)%,无坏死56例Ki-67均值为(30.27±3.632)%,两组Ki-67指数差异性显著(P=0.007,0.05);肿瘤边缘清晰组与边界毛糙组之间Ki-67表达的差异无统计学意义(P=0.106,0.05);Ki-67指数与肿瘤大小、ADC值之间无显著相关性(P0.05)。结论在浸润性乳腺癌的MRI形态学特征中,肿块坏死及腋窝淋巴结转移对于预测肿瘤细胞增殖指数Ki-67表达有重要意义。
[Abstract]:Objective to investigate the correlation between the expression of proliferative index (Ki-67) in invasive breast cancer cells and MRI morphology and apparent diffusion coefficient (MRI).Methods Eighty-five patients with invasive breast cancer between September 2013 and December 2015 were studied. All patients underwent routine MRI, diffusion weighted imaging (DWI) and dynamic enhanced scanning before operation.Results the Ki-67 index of 85 cases of invasive breast cancer was 5.70%, including 58 cases with high expression and 27 cases with low expression.There was no significant difference in Ki-67 expression between clear margin group and rough border group. There was no significant correlation between Ki-67 index and tumor size and ADC value.Conclusion among the morphologic features of MRI in invasive breast cancer, mass necrosis and axillary lymph node metastasis play an important role in predicting the expression of tumor cell proliferation index (Ki-67).
