SPG6型遗传性痉挛性截瘫的颅脑MRI表现

【摘要】:目的讨论SPG6型遗传性痉挛性截瘫(HSP)患者的颅脑核磁共振成像的表现特点,进一步提高对该病的认知与了解。 资料与方法分析12例已基因诊断为SPG6型HSP患者的颅脑MRI表现,并在TIWI的正中矢状位层面测量胼胝体膝部宽度、胼胝体体部宽度、胼胝体压部的宽度、胼胝体的面积和脑内大脑的面积。另选取400例在同一台磁共振机上行颅脑磁共振成像检查的患者作为对照组,测量方法同病例组。分别计算病例组与对照组胼胝体膝部宽度、胼胝体压部宽度、胼胝体体部宽度、胼胝体各部宽度之和、胼胝体面积、脑内大脑面积及胼脑比值的均值,最终将二者的均值进行比较。 结果2例HSP患者颅脑MRI未见明显异常。10例HSP患者出现不同程度、不同部位胼胝体变薄及脑萎缩表现,其中6例伴有双侧半卵圆中心和(或)放射冠区出现异常高信号,还有1例年长者右侧顶叶显示异常高信号。胼胝体膝部宽度、胼胝体压部宽度、胼胝体体部宽度、胼脑比值的均值患者组小于对照组,差异均有统计学意义(P005)。 结论SPG6型HSP患者的颅脑MRI可以表现出胼胝体变薄、脑萎缩、双侧半卵圆中心或放射冠区异常高信号的特点。单纯型HSP患者和复杂型HSP患者的颅脑MRI可能具有相似的特点。
[Abstract]:Objective to discuss the features of craniocerebral magnetic resonance imaging (MRI) in patients with SPG6 type hereditary spastic paraplegia.Materials and methods the craniocerebral MRI findings of 12 patients with SPG6 type HSP diagnosed by gene were analyzed. The width of the knee part of the corpus callosum, the width of the corpus callosum and the width of the splenium of the corpus callosum were measured on the median sagittal plane of TIWI.The area of the corpus callosum and the area of the brain.In addition, 400 patients were selected as control group who were examined by uplink craniocerebral magnetic resonance imaging on the same magnetic resonance machine.The genu width of corpus callosum, the width of corpus callosum, the width of corpus callosum, the total width of corpus callosum, the area of corpus callosum, the area of brain and the ratio of callosal brain were calculated.Finally, the average value of the two is compared.Results there were no obvious abnormalities in craniocerebral MRI in 2 cases of HSP. 10 cases of HSP showed different degrees of thinning of corpus callosum and brain atrophy in different parts. Among them, 6 cases were accompanied by abnormal high signal intensity in bilateral semicircular center and / or radiation-coronal area.Another patient showed abnormal hyperintensity in the right parietal lobe.The genu width of the corpus callosum, the width of the corpus callosum, the width of the corpus callosum and the ratio of callosal brain in the patients group were smaller than those in the control group (P 0.05).Conclusion craniocerebral MRI in patients with SPG6 type HSP can be characterized by thinning of corpus callosum, atrophy of brain, abnormal hyperintensity of bilateral semicircular center or radio-coronal area.Craniocerebral MRI may have similar characteristics in patients with simple HSP and complex HSP.
