超声在钙化性甲状腺良恶性结节鉴别诊断中的价值

【摘要】：目的研究超声在甲状腺良性钙化性结节(BCNS)和恶性钙化性结节(MCNS)鉴别诊断中的价值。方法回顾经病理证实的392例464枚甲状腺钙化性结节的超声表现,分析微钙化、粗钙化、环状钙化、混合钙化、微钙化+混合钙化在BCNS和MCNS中的分布。结果 464枚甲状腺钙化性结节中,382枚BCNS,82枚MCNS,微钙化、微钙化+混合钙化及粗钙化在二者之间具有统计学差异,微钙化及微钙化+混合钙化更常见于MCNS中,粗钙化更常见于BCNS中;环状钙化及混合钙化在二者之间无统计学意义。结论微钙化有利于MCNS的诊断,粗钙化提示BCNS的诊断,尽管混合钙化在二者之间无统计学意义,但将其并入微钙化,可明显提高MCNS诊断的灵敏度和阴性预测值,减少MCNS漏诊的发生率。关键词
[Abstract]:Objective to study the value of ultrasonography in differential diagnosis of benign calcified thyroid nodules (BCNS) and malignant calcified nodules (MCNS).Methods the ultrasonographic findings of 464 thyroid calcified nodules in 392 cases confirmed by pathology were retrospectively reviewed. The distribution of microcalcification, coarse calcification, circular calcification, mixed calcification and mixed calcification in BCNS and MCNS was analyzed.Results there were significant differences in microcalcification, mixed calcification and coarse calcification between 464 thyroid calcified nodules with 382 BCNSs and 82 MCNSs. Microcalcification and mixed calcification were more common in MCNS, and coarse calcification was more common in BCNS.There was no significant difference between circular calcification and mixed calcification.Conclusion Microcalcification is beneficial to the diagnosis of MCNS. Coarse calcification suggests that the diagnosis of BCNS, although mixed calcification is not statistically significant between the two, but its incorporation into microcalcification can significantly improve the diagnostic sensitivity and negative predictive value of MCNS.To reduce the incidence of MCNS missed diagnosis.Keywords
