【摘要】:目的通过应用超声方法探讨小儿发育性髋关节发育不良(DDH)与先天性肌性斜颈(CMT)早期筛查的意义,建立小儿DDH与CMT的同步筛查模式,提高DDH患儿的确诊率。方法对2014年1月~2017年1月于我院就诊的5060例疑似DDH与CMT小儿进行常规超声检查,对两种疾病的诊断结果按照临床分型,不同DDH分型与不同CMT分型间是否有关联性单因素采用χ~2检验,多因素采用对应分析法进行统计分析;疑似CMT就诊来源小儿中CMT患儿与颈部正常小儿在DDH检出率上的比较采用χ~2检验。结果不同DDH分型与不同CMT分型间有关联性(χ2=331.800,P0.001),对应分析结果表明GrafⅡa型DDH小儿与肿块型CMT小儿有关联,GrafⅡb型、GrafⅡc型、GrafⅢ型、GrafⅣ型DDH小儿与非肿块型CMT小儿有关联;疑似CMT就诊小儿中CMT患儿的DDH检出率显著高于颈部正常小儿(χ~2=321.889,P0.001)。结论 DDH与CMT的发病有较大的关联性,应尽早在临床上开展DDH与CMT的同步筛查,有助有提高DDH患儿的早期诊断率。
[Abstract]:Objective to explore the significance of early screening of developmental hip dysplasia (DDH) and congenital muscular torticollis (CMT) in children by using ultrasound, and to establish a synchronous screening model of DDH and CMT in children, so as to improve the diagnosis rate of children with DDH.Methods from January 2014 to January 2017, 5060 children with suspected DDH and CMT were examined by conventional ultrasound. The diagnostic results of the two diseases were classified according to clinical classification.Whether there was correlation between different DDH typing and different CMT typing was analyzed by 蠂 ~ 2 test and multivariate correspondence analysis.蠂 2 test was used to compare the detection rate of DDH between children with CMT and normal children with neck in suspected CMT.Results there was a correlation between different DDH classification and different CMT classification (蠂 ~ 2 ~ (2) n ~ (331.800)) (P 0.001).
