双指数模型DWI预测鼻咽癌分化程度的初步研究

【摘要】:目的:探讨双指数模型磁共振扩散加权成像(DWI)对鼻咽癌(NPC)分化程度的预测价值。方法:根据肿瘤的分化程度,将33例经病理证实的非角化型NPC患者分为分化组(14例)和未分化组(19例)。所有患者均行双指数模型DWI检查,比较2组中DWI参数值(ADCstand、ADCslow、ADCfast和Ffast值)的差异。结果:分化型NPC的ADCstand、ADCslow、ADCfast和Ffast值分别为(0.920±0.131)×10-3 mm2、(0.680±0.115)×10-3 mm2、(22.3±13.9)mm2/s和0.316±0.078;未分化型NPC的ADCstand、ADCslow、ADCfast和Ffast值分别为(0.952±0.203)×10-3 mm2/s、(0.672±0.116)×10-3 mm2/s、(17.8±20.3)mm2/s和0.299±0.120;仅ADCfast值在两组间差异有统计学意义(P0.05)。ADCfast值预测NPC分化程度的敏感度为85.7%,特异度为63.2%。结论:ADCfast值对预测鼻咽癌的分化程度具有一定价值。
