VBM对首发未经治疗抑郁症患者脑结构改变的研究

【摘要】:[目的]运用基于体素的形态学测量方法分析首发未经治疗抑郁症患者脑灰质变化,探讨抑郁症脑结构改变的神经病理生理学机制。[方法]收集首发抑郁症患者27例(MDD组)及与MDD组性别、年龄及受教育程度匹配的健康志愿者30名;所有患者由两名以上临床心理科医生根据美国《精神疾病诊断与统计手册》第四版(Diagnostic and statistical manual,fourth edition DSM-IV)诊断标准确诊。采用美国GE公司1.5T Signa(HDXT)双梯度磁共振扫描仪,头部相控阵线圈进行常规头颅磁共振、全脑3D结构及功能磁共振扫描,获取被试者全脑相应数据,采用基于体素的形态测量(voxel-based morphometry,VBM)方法对两组的脑结构MRI图像的灰质体积分析,采用两独立样本t检验对两组结果进行比较,当p0.05时认为差异有统计学意义。[结果](1)抑郁症组与健康组在性别、年龄及受教育年限上差异无统计学意义。(2)VBM分析显示首发抑郁症组较正常对照组灰质体积减小的脑区:右侧颞上回、右侧颞中回、右侧颞下回(P0.001,cluster≥12)。增加的脑区:左侧额上回、左侧额中回、左侧额下回、左前扣带回、右侧额下回(P0.001,cluster≥12)。[结论]抑郁症患者脑灰质体积较健康人异常的脑区,主要涉额叶、颞叶及扣带回,可能是抑郁症发病的神经生理病理学机制的关键。
[Abstract]:[objective] to investigate the neuropathophysiological mechanism of the changes of brain structure in patients with depression without initial treatment by means of morphometric measurement based on voxel. [methods] 27 patients with first-episode depression (MDD group) and 30 healthy volunteers matched with sex, age and education level of MDD group were collected. All patients were diagnosed by more than two clinical cardiologists according to the diagnostic criteria of Diagnostic and statistical fourth edition DSM-IV. Two gradient magnetic resonance scanners of 1.5T SignaAn HDXT of GE Company were used in this study. The head phased array coil was used to perform conventional cranial magnetic resonance imaging, and 3D structure and functional magnetic resonance scanning of the whole brain were performed to obtain the corresponding data of the whole brain of the subjects. A voxel-based morphometrymetric method was used to analyze the gray matter volume of MRI images of brain structure in two groups. Two independent samples t-test were used to compare the results of the two groups. When p0.05, the difference was statistically significant. [results] there was no significant difference between the depression group and the healthy group in sex, age and years of education. VBM analysis showed that the gray matter volume of the first episode depression group was smaller than that of the normal control group: right superior temporal gyrus, right middle temporal gyrus, right superior temporal gyrus, and right middle temporal gyrus.
