股骨头缺血坏死的MRI分期与坏死指数的关系研究

【摘要】:目的: 采用MRI扫描图像,探讨股骨头缺血性坏死在磁共振上的分期与坏死指数之间的相关性,并寻找股骨头塌陷的临界点,以期为临床利用MRI图像预测股骨头塌陷提供更全面的参考标准。 方法: 回顾分析2012年7月至2014年1月69例(共100髋)在我科经磁共振检查可确诊为股骨头缺血性坏死而且图像可以显示明确分期的病患,对每一个缺血性坏死股骨头的MRI图像依照Hungerford和Lennox制定的五期分法进行分期,另采用Koo等提出的坏死指数法计算公式算出每一个股骨头的坏死指数,将分期与坏死指数对应起来,运用统计学的方法对二者进行分析,探讨二者间存在的关系,并确定股骨头塌陷的临界点。 结果: Ⅰ期股骨头缺血性坏死34髋,坏死指数为0.32±0.20(95%CI:0.25~0.39),Ⅱ期股骨头缺血性坏死32髋,坏死指数均值为0.49±0.17(95%CI:0.43~0.55),Ⅲ期股骨头缺血性坏死18髋,坏死指数均值为0.62±0.18(95%CI:0.53~0.71),Ⅳ期股骨头缺血性坏死16髋,坏死指数均值为0.83±0.23(95%CI:0.70~0.95),各期股骨头坏死指数经显著性检验,差异有统计学意义(P0.05),snk法对任意两期坏死指数均值差的统计学分析结果显示工期与Ⅱ期坏死指数均值差为-0.17,Ⅰ期与Ⅲ期坏死指数均值差为-0.30,Ⅰ期与Ⅳ期坏死指数均值差为-0.51,Ⅱ期与Ⅲ期坏死指数均值差为-0.13,Ⅱ期与Ⅳ期坏死指数均值差为-0.33,Ⅲ期与Ⅳ期坏死指数均值差为-0.21,利用SNK法对以上任意两期均值差进行分析,结果显示任意两组差异均有统计学意义(P0.05)。Ⅱ期(无塌陷)仅3髋坏死指数0.53,Ⅲ期(开始塌陷)各髋坏死指数均0.53。 结论: 在MRI图像上,股骨头缺血性坏死的坏死指数随着分期推进而增大;坏死指数0.53可作为股骨头塌陷的参考临界值,在MRI分期不明确的情况下,结合坏死指数可以为临床预测股骨头塌陷提供更全面的信息。
[Abstract]:Objective: The correlation between MRI staging and necrosis index of avascular necrosis of femoral head was studied by using MRI scanning image, and the critical point of collapse of femoral head was found. In order to provide a more comprehensive reference standard for clinical prediction of femoral head collapse using MRI images. Methods: From July 2012 to January 2014, 69 cases (100 hips) with avascular necrosis of the femoral head diagnosed by MRI in our department were analyzed retrospectively. The MRI images of each avascular necrosis of femoral head were divided into five stages according to Hungerford and Lennox. The necrosis index of each femoral head was calculated by using the formula of necrosis index proposed by Koo et al. The statistical method was used to analyze the relationship between them and to determine the critical point of the collapse of the femoral head.
