容易误诊的高回声型乳腺病变

【摘要】:目的分析超声乳腺影像报告和数据系统(BI-RADS)分类4类及以上乳腺占位性病变中高回声病变的发生率及其中乳腺癌所占比例,探讨乳腺高回声占位性病变的超声表现与其组织病理学结构的关系,减少误诊。方法回顾性分析2012年6月至2014年3月间在北京大学第三医院接受经超声引导下穿刺活检或手术取得病理结果的848例乳腺占位性病变,筛选出其中的高回声病变,对病变的超声表现进行分析并统计高回声病变所占比例及其中乳腺癌的发生率。结果848例乳腺占位性病变中,高回声表现者8例(0.9%),其中乳腺良性病变6例,乳腺癌2例。高回声乳腺癌在高回声病变中所占比例高达25%(2/8),在全部乳腺癌中的比例为0.7%(2/280)。乳腺高回声病变存在炎性水肿、陈旧积乳、大量砂砾体等病理基础,但良恶性病变在形态、回声特征上无明显差异。结论乳腺高回声占位性病变及高回声型乳腺癌尽管少见,但高回声型乳腺癌在高回声占位性病变中的发生比例并不低,值得重视并需结合病史及其他方法进行鉴别。
[Abstract]:Objective to analyze the incidence of hyperechoic lesions in 4 types of breast space occupying lesions and the proportion of breast cancer in 4 categories and above, and to explore the relationship between ultrasound and histopathological structure of hyperechoic lesions of the breast, and to reduce misdiagnosis. Methods Retrospective analysis from June 2012 to 20 In March, 848 cases of breast space occupying lesions were selected by ultrasound guided biopsy or surgery in No.3 Hospital of Peking University between 14 years. The hyperechoic lesions were screened out. The ultrasonic manifestations of the lesions were analyzed and the proportion of hyperechoic lesions and the incidence of middle breast cancer were statistically analyzed. Results 848 cases of breast space occupying lesions were found. Among the lesions, 8 cases (0.9%) were hyperechoic, including 6 cases of benign breast lesions and 2 cases of breast cancer. The proportion of hyperechoic breast cancer in hyperechoic lesions was 25% (2/8), and the proportion in all breast cancers was 0.7% (2/280). The pathological basis of hyperechoic lesions of the breast was inflammatory edema, old milk, and large amount of gravel, but the benign and malignant lesions were in the breast. Conclusion hyperechoic breast cancer and hyperechoic breast cancer are rare, but the proportion of hyperechoic breast cancer in high echo space occupying lesions is not low. It is worthy of attention and should be identified in combination with medical history and other methods.
