速度向量成像评价高血压患者颈动脉粥样硬化斑块稳定性的研究

【摘要】:目的应用速度向量成像技术比较高血压患者与非高血压患者颈总动脉粥样硬化斑块运动特性及斑块力学特点,探讨高血压患者与非高血压患者的颈动脉斑块稳定性是否存在差异。 方法回顾性分析93例颈动脉粥样硬化斑块患者的速度向量成像资料,其中51例患者伴有原发性高血压,其余42例患者血压正常,应用速度向量成像(VVI)技术检测颈总动脉粥样硬化斑块基底部、肩部、纤维帽顶部的运动速度、应变率,利用卡方检验分组进行比较分析。 结果51例高血压组患者共检出75处粥样硬化斑块,其中52处硬斑块,23处软斑块,42例非高高血压组患者共检出56处粥样硬化斑块,其中39处硬斑块,17处软斑块,高血压组颈动脉软斑块肩部近心端、顶部收缩期最大运动速度、最大应变率高于非高血压组,差异有统计学意义(P0.05或P0.01),而软斑块肩部远心端、基底部收缩期最大运动速度、最大应变率与非高血压组比较差别无明显统计学意义(P0.05);高血压组颈动脉硬斑块肩部近心端、远心端、顶部、基底部收缩期最大运动速度、最大应变率略高于非高血压组,差异无明显统计学意义(P0.05)。 结论速度向量成像技术可用于高血压患者颈动脉粥样硬化斑块及斑块不同部位运动速度、应变及应变率的分析,为评价斑块稳定性提供新的方法;高血压患者颈动脉软斑块稳定性低于非血压患压患者,而硬斑块稳定性两组患者差别不明显。
[Abstract]:Objective to compare the motion and mechanical characteristics of carotid atherosclerotic plaques between hypertensive patients and non-hypertensive patients by velocity vector imaging. To investigate whether carotid plaque stability is different between hypertensive patients and non-hypertensive patients. Methods the velocity-vector imaging data of 93 patients with carotid atherosclerotic plaque were analyzed retrospectively. Among them, 51 cases were accompanied with essential hypertension, and 42 cases had normal blood pressure. Velocity vector imaging (VVI) technique was used to detect the velocity and strain rate of carotid atherosclerotic plaques at the base, shoulder and top of the fibrous cap. The chi-square test was used to compare and analyze the velocity and strain rate of carotid atherosclerotic plaques. Results Seventy-five atherosclerotic plaques were detected in 51 patients with hypertension, including 52 hard plaques, 23 soft plaques, 42 non-hypertensives, 56 atherosclerotic plaques, 39 hard plaques and 17 soft plaques. In the hypertension group, the maximum systolic velocity and strain rate at the top of the shoulder were significantly higher than those in the non-hypertensive group (P0.05 or P0.01), while the maximal systolic velocity in the distal shoulder and the base of the soft plaque was higher than that in the non-hypertensive group (P0.05 or P0.01). There was no significant difference between the maximum strain rate and the non-hypertension group (P0.05). The maximum systolic velocity of the shoulder, distal end, top and base of the carotid artery hard plaque in the hypertension group was slightly higher than that in the non-hypertension group, and the maximum strain rate was higher than that in the non-hypertension group. The difference was not statistically significant (P0.05). Conclusion Velocity vector imaging can be used to analyze the velocity, strain and strain rate of carotid atherosclerotic plaques and different parts of plaque in hypertensive patients, which provides a new method for evaluating plaque stability. The stability of carotid soft plaque in hypertensive patients was lower than that in patients without blood pressure, but there was no significant difference in the stability of hard plaques between the two groups.
