轻度认知功能障碍脑白质扩散峰度成像研究

【摘要】:目的:利用扩散峰度成像技术探究轻度认知功能障碍患者脑白质微细结构的改变,为其发病机制研究提供有价值的影像信息。方法:收集临床上确诊为轻度认知功能障碍的患者20例,健康志愿者20例,行常规MRI平扫及DKI技术扫描,测量双侧额叶、顶叶、枕叶、颞叶、扣带回后部及胼胝体压部的FA值、MD值、MK值;针对有差异的感兴趣区,分析其与年龄、受教育年限的相关性。结果:病例组患者左侧额叶、左侧顶叶、双侧颞叶、双侧后扣带回及双侧枕叶FA值均低于健康对照组,且左侧顶叶FA值的变化在不同的年龄组有统计学差异。病例组患者左侧额叶、右侧顶叶、左侧颞叶、左侧后扣带回及双侧枕叶MD值均高于健康对照组。病例组患者双侧颞叶、左侧后扣带回MK值均低于健康对照组,且左侧扣带回后部MK值的变化在男性与女性间有统计学差异。病例组患者双侧枕叶FA值与年龄呈负相关、与受教育年限呈正相关,左侧枕叶MD值与年龄呈正相关、与受教育年限呈负相关,差异有统计学意义。结论:扩散峰度成像技术为探究轻度认知功能障碍脑白质损害的发病机制提供了有价值的信息,其脑白质损害的部位具有一定的选择性。
[Abstract]:Aim: to explore the changes of white matter microstructures in patients with mild cognitive impairment by diffusion kurtosis imaging (DKI), and to provide valuable imaging information for the study of pathogenesis. Methods: twenty patients with mild cognitive impairment and 20 healthy volunteers were examined by conventional MRI scan and DKI technique. Bilateral frontal lobe, parietal lobe, occipital lobe and temporal lobe were measured. FA value and MK value of posterior cingulate gyrus and corpus callosum were analyzed. Results: the FA values of left frontal lobe, left parietal lobe, bilateral temporal lobe, bilateral posterior cingulate gyrus and bilateral occipital lobe in the case group were lower than those in the healthy control group, and the changes of FA value in the left parietal lobe were statistically different in different age groups. The MD values of left frontal lobe, right parietal lobe, left temporal lobe, left posterior cingulate gyrus and bilateral occipital lobe in the case group were higher than those in the healthy control group. The MK values of bilateral temporal lobe and left posterior cingulate gyrus in the case group were lower than those in the healthy control group, and the change of MK value in the left posterior cingulate gyrus was statistically different between male and female. The FA value of bilateral occipital lobe was negatively correlated with age, positively correlated with years of education, MD value of left occipital lobe was positively correlated with age and negatively correlated with years of education, the difference was statistically significant. Conclusion: diffusion kurtosis imaging provides valuable information for the study of the pathogenesis of mild cognitive impairment in the white matter, and the location of the white matter damage is selective.
