应用二维及实时三维超声心动图评价正常人左房构型及功能的研究

【摘要】:第一部分应用左房追踪技术探讨正常人左房参数及影响因素 目的:应用左房追踪技术探讨正常人左房参数及其影响因素。 方法:选取202例正常人,其中男99例、女103例。男、女平均年龄分别为(41.5±12.5)岁、(44.9±13.1)岁,运用左房追踪技术(left atrial tracking, LAT)获得左房:最大上下径(LALmax)、最小上下径(LALmin)、最大周长(LACmax)、最小周长(LACmin)、最大面积(LAAmax)、最小面积(LAAmin)、最大容积(LAVmax)、最小容积(LAVmin)、收缩前容积(LApre)、收缩期充盈速率(dv/dtS)、左房舒张早期排空速率(dv/dtE)、左房舒张晚期排空速率(dv/dtA)参数的正常参考值范围,并用体表面积(BSA)标化,研究其是否受性别、年龄因素影响。 结果:男性组与女性组比较,左房各参数均值差异无统计学意义(P㧐0.05)。左房各参数在不同年龄组之间差异具有统计学意义(P0.05),各参数随年龄的增长而增加。 结论:左房追踪技术测得正常人左房各参数随着年龄的增长而增大,但与性别无关,作为新技术在探讨正常人左房构型及功能方面具有一定的可行性。 第二部分应用实时三维超声心动图对正常人左房功能相关参数的研究 目的:应用实时三维超声心动图(real-time three-dimensionalechocardiography,RT-3DE)探讨正常人左房容积及功能相关参数的正常值及其应用价值。 方法:选取202例健康志愿者,按性别分为男性组和女性组,按年龄分为A组≤40岁(63例),B组41~60岁(87例),C组㧐60岁(52例)。应用RT-3DE技术获得左房最大容积(LAEDV)、左房最小容积(LAESV)、左房射血量(LASV)、左房收缩前容积(LApre)、左房被动排空容积(LAVp)、左房主动排空容积(LAVa)和左心房射血分数(LAEF),并对参数用体表面积校正,得出LAEDVI、LAESVI、LASVI、LApreI、LAVpI、LAVaI、LAEF’,统计分析得到上述参数正常值区间范围,并比较各年龄组及性别组之间的差异。 结果:202例健康志愿者LAEDVI为(32.58~35.05)ml/m2,LAESVI为(14.80~16.21)ml/m2,LASVI为(17.87~19.40)ml/m2,LApreI为(17.99~19.86)ml/m2、LAVpI为(10.46~11.92)ml/m2、LAVaI为(6.88~7.92)ml/m2、LAEF’为(53.54~55.95)%。男性组与女性组比较LAEDVI、LAESVI、LASVI、LApreI、LAVpI、LAVaI、LAEF’值差异无统计学意义(P0.05)。不同年龄组间比较LAEDVI、LAESVI、LASVI、LApreI、LAVpI、LAVaI值差异有统计学意义(P0.05),LAEF’值在各年龄组间呈递减趋势,但差异无统计学意义(P0.05)。结论:左房各参数值与性别无关,,LAEDVI、LAESVI、LASVI、LApreI、LAVpI、LAVaI随年龄增加而增大,LAEF’不随年龄变化。RT-3DE技术具有实时、动态、立体、准确、无创等优点,在评价左房容积及功能方面是可行的,为临床能够正确评估左房功能提供了新方法。
[Abstract]:The first part was to study the parameters of left atrium and its influencing factors in normal subjects by using left atrial tracing technique. Objective: to explore the parameters of left atrium and its influencing factors by using the technique of left atrial tracing. Methods: 202 normal persons were selected, including 99 males and 103 females. The mean age of male and female were (41.5 卤12.5) years and (44.9 卤13.1) years, respectively. Using left atrial tracing technique (left atrial tracking, LAT) to obtain left atrium: maximum inferior and inferior diameter (LALmax), minimum upper and lower diameter (LALmin), maximum circumference (LACmax), minimum circumference (LACmin), maximum area minimum area (LAAmin), maximum volume (LAVmax), minimum volume (LAVmin), systolic volume (LApre), systolic filling The normal reference range of the parameters of left atrial early diastolic emptying rate (dv/dtE), left atrial late diastolic emptying rate (dv/dtA), left atrial velocity (dv/dtS), left atrial early diastolic velocity (dv/dtE), left atrial late diastolic velocity (dv/dtA), The body surface area (BSA) was standardized to study whether it was affected by sex and age. Results: there was no significant difference in the mean values of left atrial parameters between male group and female group (P 0.05). The parameters of left atrium were significantly different among different age groups (P0.05), and the parameters increased with the increase of age. Conclusion: the parameters of left atrium in normal subjects increased with age, but not related to sex. As a new technique, it is feasible to study the configuration and function of left atrium in normal subjects. Part two: study on parameters related to left atrial function in normal subjects by real-time three-dimensional echocardiography objective: to explore the normal parameters of left atrial volume and function in normal subjects by using real-time three-dimensional echocardiography RT-3DE Value and its application value.
