弹性成像技术在浆细胞性乳腺炎鉴别诊断中的价值

【摘要】:目的分别研究31例浆细胞性乳腺炎的常规超声及弹性成像超声表现,分析二者的不同,探讨超声弹性成像技术在浆细胞性乳腺炎诊断及鉴别诊断中的作用和价值,提高超声弹性成像技术在浆细胞性乳腺炎诊断中的确诊率从而降低误诊率。研究方法回顾性分析2010年2月到2013年12月期间在邹平县人民医院就诊并接受手术或活检穿刺后病理证实为浆细胞性乳腺炎的31例患者31处病灶的超声表现,常规超声观察病造部位、大小、形态、边界、包膜、内部回声、血流等情况,并对所查病灶进行BI-RADS分级,4分及4分以上诊断为恶性,3分及3分以下诊断为良性,考虑浆细胞性乳腺炎。然后对病灶处进行弹性成像检查,获得较稳定、清晰的弹性图像后分析弹性成像图,并对弹出成像图采用改良5分法进行评分。分析弹性成像在浆细胞性乳腺炎中的应用价值。结果本组研究中,31例浆细胞性乳腺炎常规超声根据BI-RADS分级误诊为恶性病变7例,常规超声联合弹性成像检查根据弹性评分法误诊为恶性病变3例。结论1、浆细胞性乳腺炎,尤其是囊实性包块型及实性包块型,声像图与乳腺癌相似,常规超声易将其误诊为乳腺癌。2、常规超声联合弹性成像技术能有效提高浆细胞性乳腺炎囊实性包块型及实性包块型的诊确诊率,从而降低误诊率。3、在浆细胞性乳腺炎诊断中,超声弹性成像技术还不够全面及完善,存在一定的假阴性或假阳性,该技术仍有待于进一步研究及完善。
[Abstract]:Objective to study the ultrasonographic features of 31 cases of plasmacytic mastitis, analyze the difference between them, and discuss the value of ultrasonic elastography in the diagnosis and differential diagnosis of plasmacytic mastitis. To improve the diagnostic accuracy of ultrasonic elastography in the diagnosis of plasma cell mastitis and reduce the misdiagnosis rate. Methods from February 2010 to December 2013, 31 patients with plasmacytic mastitis, who were treated at Zouping County people's Hospital and confirmed by biopsy or operation, were analyzed retrospectively. The location, size, shape, boundary, capsule, internal echo and blood flow of the lesions were observed by conventional ultrasound. The lesions were classified by BI-RADS, 4 or more were diagnosed as malignant, and 3 or less were diagnosed as benign. Consider plasma cell mastitis. Then elastic imaging was performed on the lesion to obtain a more stable and clear elastic image post-analysis elastography, and the improved 5 score method was used to score the ejection image. To analyze the application value of elastic imaging in plasma cell mastitis. Results in this study, 31 cases of plasmacytic mastitis were misdiagnosed as malignant lesions by conventional ultrasound according to BI-RADS classification, and 3 cases were misdiagnosed as malignant lesions by conventional ultrasound combined with elastic imaging according to elastic score. Conclusion 1. Plasma cell mastitis, especially cystic mass and solid mass, are similar to breast cancer in sonogram. Conventional ultrasound combined with elastic imaging can effectively improve the diagnostic accuracy of cystic mass type and solid mass type of plasmacytic mastitis, thus reducing the misdiagnosis rate of 0.3, in the diagnosis of plasmacytic mastitis. Ultrasonic elastic imaging technology is not comprehensive and perfect, there are some false negative or false positive, this technology still needs to be further studied and improved.
