乳腺浸润性导管癌超声图像特征与上皮—间叶转化的关键因子Twist及E-cadherin表达的相关性研究

【摘要】:目的: 探讨乳腺浸润性导管癌(infiltrating ductal carcinoma, IDC)超声声像图特征与上皮间叶转化的关键性因子Twist、E-cadherin表达的相关性。旨在为浸润型导管癌的恶性程度判定和预后评估提供一定的指导意义。 方法: 选择经术后病理诊断证实为浸润型导管癌的乳腺癌标本40例,所有患者术前未经过任何形式的放化疗,入选病例术前均经彩色多普勒超声检查,采用免疫组织化学EnVision法检测Twist、E-cadherin蛋白的表达情况。将其声像图特征(肿瘤大小、血流分级、肿物边缘如毛刺征、微小钙化、后方回声衰减、纵横比、同侧腋窝淋巴结肿大及周围软组织侵犯等)与Twist、E-cadherin的表达情况进行比较,分析超声影像特征与二者表达的相关性。 结果: 1.40例乳腺浸润型导管癌中,超声图像肿瘤≥2cm31例,血流分级2-3级35例,边缘不完整,有毛刺或小分叶者29例,纵横比≥1者22例,同侧腋窝淋巴结肿大25例,微小钙化31例,周围组织侵犯14例,后方回声衰减16例。 2.40例乳腺浸润型导管癌中,Twist蛋白阳性表达29例(72.5%),显著高于癌旁正常组织(15.0%),差异有统计学意义(p0.05)。E-cadherin阳性表达22例(55.0%),显著低于癌旁正常组织(100.0%),差异有统计学意义(p0.05)。 3. Twist在肿瘤边缘不完整、同侧腋窝淋巴结肿大、周围软组织侵犯中的阳性表达显著高于边缘完整、无同侧腋窝淋巴结肿大及未侵犯周围软组织者(p0.05)。E-cadherin在肿瘤边缘不完整、周围软组织侵犯中的阳性表达显著低于边缘完整及未侵犯周围软组织者(p0.05)。 结论: 1.乳腺浸润性导管癌可表现为不同的超声图像及组织病理学特征。 2.浸润性导管癌的超声图像与上皮间叶转化的标志性因子Twist、E-cadherin的表达存在一定的关联性,两者结合在一定程度上可以判断肿瘤侵袭、转移等生物学行为,为浸润性导管癌的恶性程度及预后判断具有一定的指导意义。
[Abstract]:Objective: to investigate the correlation between (infiltrating ductal carcinoma, IDC) sonographic features and the expression of Twist,E-cadherin, a key factor of interepithelial lobe transformation in invasive ductal carcinoma of the breast. The purpose of this study is to provide guidance for the evaluation of malignant degree and prognosis of invasive ductal carcinoma. Methods: 40 cases of breast cancer with invasive ductal carcinoma confirmed by pathological diagnosis after operation were selected. All the patients did not undergo any form of radiotherapy and chemotherapy before operation. All the selected cases were examined by color Doppler ultrasound before operation. Immunohistochemical EnVision method was used to detect the expression of Twist,E-cadherin protein. The ultrasonographic features (tumor size, blood flow grade, margin of tumor such as burr sign, microcalcification, posterior echo attenuation, aspect ratio, ipsilateral axillary lymph node enlargement and surrounding soft tissue invasion, etc.) were compared with the expression of Twist,E-cadherin.
