健康志愿者肾脏ADC值测量及其影响因素的初步研究

【摘要】:目的:探讨年龄和性别因素对健康志愿者肾脏ADC值的影响。材料与方法:2012年6月至2013年8月共100名健康志愿者纳入本研究(其中男女各50名,年龄18-69岁,平均44岁),所有志愿者肾功能正常、没有泌尿系统疾病和高血压等基础病。根据年龄分为5个组(18-29岁,30-39岁,40-49岁,50-59岁和60-69岁),每组各20名,在3.0T磁共振扫描仪上行肾脏MR常规平扫和RT-DWI成像(b值采用800s/mm2),测量肾实质ADC值并进行统计学分析。结果:1、5个年龄段肾实质ADC值依次为(2.066±0.099)×10-3mm2/s、(2.059±0.124)×10-3mm2/s、(2.034±0.103)×10-3mm2/s、(1.985±0.172)×10-3mm2/s、(1.943±0.125)×10-3mm2/s,各组间具有统计学差异(F=3.375,P0.05)。2、5个不同年龄段男性肾实质ADC值依次为(2.087±0.094)×10-3mm2/s、(2.086±0.146)×10-3mm2/s、(2.042±0.117)×10-3mm2/s、(2.041±0.205)×10-3mm2/s、(1.979±0.122)×10-3mm2/s;女性肾实质ADC值依次为(2.060±0.126)×10-3mm2/s、(2.031±0.097)×10-3mm2/s、(2.026±0.092)×10-3mm2/s、(1.929±0.115)×10-3mm2/s、(1.907±0.124)×10-3mm2/s。5个不同年龄组男性平均ADC值均大于女性,差异没有统计学意义(P0.05)。结论:健康志愿者肾脏ADC值受年龄和性别的影响:伴随年龄的增长,健康志愿者肾实质ADC值呈减小的趋势;同一年龄段男性和女性ADC值无显著差别。因此,DWI成像在临床应用时应考虑到年龄和性别因素对ADC值的影响。 第二部分健康志愿者肾脏1.5T和3.0T磁共振扩散加权成像的对比研究 目的:探讨不同场强(1.5T和3.0T)对ADC值测量和图像质量的影响。材料与方法:2012年6月至2013年12月共150名健康志愿者(其中男女各75名,年龄18-73岁,平均38岁)纳入本研究,根据年龄分为5个组(18-29岁,30-39岁,40-49岁,50-59岁和60-69岁),每组各30名,同一志愿者分别在 1.5T与3.0T磁共振扫描仪上进行肾脏FB-DWI和RT-DWI成像(b值均采用800s/mm2)。通过配对t检验比较两种场强DWI图像的信噪比(SNR),通过单因素方差分析进行比较肾脏不同部位(上、中和下部)ADC值差异,通过Pearson相关分析比较不同场强ADC值测量的一致性。结果:1、FB-DWI图像3.0T和1.5T MR的SNR分别为(66.500±14.776)、(51.570±8.578),差异具有统计学意义(P0.05);RT-DWI图像3.0T和1.5T MR的SNR分别为(66.611±20.748)、(44.989±11.461),差异具有统计学意义(P0.05)。 2、1.5T MR肾脏上部、中部和下部的ADC值分别为(1.998±0.102)×10-3mm2/s、(2.157±0.166)×10-3mm2/s、(2.050±0.183)×10-3mm2/s,3.0T MR肾脏上部、中部和下部的ADC值分别为(2.042±0.094)×10-3mm2/s、(2.073±0.147)×10-3mm2/s、(1.947±0.086)×10-3mm2/s,同一场强肾脏三个部位的ADC值无统计学差异(1.5T:F=2.485,P0.05;3.0T:F=3.107,P0.05),肾脏同一部位在不同场强下ADC值无统计学差异(P0.05)。3、1.5T MR5个年龄段肾实质ADC值依次为(2.165±0.162)×10-3mm2/s、(2.145±0.157)×10-3mm2/s、(2.142±0.150)×10-3mm2/s、(2.127±0.183)×10-3mm2/s、(2.079±0.119)×10-3mm2/s, 3.0T MR5个年龄段肾实质ADC值依次为(2.075±0.123)×10-3mm2/s、(2.066±0.117)×10-3mm2/s、(2.033±0.108)×10-3mm2/s、(1.971±0.127)×10-3mm2/s、(1.942±0.099)×10-3mm2/s。4、1.5T和3.0T健康肾脏ADC值测量Pearson相关系数为0.796。结论:3.0T肾脏DWI图像信噪比高于1.5T;同一b值(b=800s/mm2)条件下,1.5T和3.0T磁共振在不同部位和不同年龄段的肾脏ADC值差别不显著,,二者具有良好的一致性。

Comparative study of 1.5T and 3.0T diffusion weighted imaging in kidney of second healthy volunteers
Materials and Methods: From June 2012 to December 2013, 150 healthy volunteers (75 males and 75 females, 18-73 years old, average 38 years old) were included in this study. They were divided into 5 groups according to their ages (18-29 years old, 30-39 years old, 40-49 years old, 50-59 years old and 60-69 years old). The same volunteer is in the same place.
FB-DWI and RT-DWI were performed on 1.5T and 3.0T magnetic resonance scanners (b values were 800s/mm2). Signal-to-noise ratios (SNR) of DWI images were compared by paired t test. The differences of ADC values in different parts of the kidney (upper, middle and lower) were compared by one-way ANOVA. Pearson correlation analysis was used to compare the ADC values measured at different field strengths. Results: 1, the SNR of FB-DWI images 3.0T and 1.5T MR were (66.500 + 14.776) and (51.570 + 8.578) respectively, the difference was statistically significant (P 0.05); the SNR of RT-DWI images 3.0T and 1.5T MR were (66.611 + 20.748) and (44.989 + 11.461), respectively, and the difference was statistically significant (P 0.05).
