应用经会阴三维超声在评价选择性剖宫产后盆底功能变化中的作用

【摘要】:目的探讨应用经会阴三维超声评价选择性剖宫产后产妇的盆底功能变化。方法选取行选择性剖宫产分娩的初产妇86例作为研究对象,于产后42 d行经会阴三维超声检查,观察静息状态及Valsalva动作时有无盆底组织异常,测量并计算膀胱颈移动度、肛提肌裂孔面积等指标。根据超声检查结果,将病例分为有盆底功能障碍组(观察组)以及无盆底功能障碍组(对照组)两组,对两组产妇的临床检查及超声检查结果进行统计学分析。结果 86例选择性剖宫产后产妇中有盆底功能障碍者(观察组)18例,无盆底功能障碍者(对照组)68例。观察组初产妇临床检查压力性尿失禁6例,阴道前壁脱垂1例,子宫脱垂1例,其余10例未检出异常。对照组初产妇临床检查压力性尿失禁9例,其余59例未检出异常。观察组初产妇经会阴三维超声检查膀胱颈活动度增加13例,阴道前壁脱垂2例,子宫脱垂2例,直肠膨出1例。对照组初产妇经会阴三维超声未检出异常。观察组初产妇在Valsalva动作时,膀胱颈移动度明显大于对照组(P0.05),肛提肌裂孔的面积明显大于对照组(P0.05),差异有统计学意义。结论经会阴三维超声检查可以早期发现轻度的盆底功能障碍,可以简便有效地评价选择性剖宫产后的盆底功能变化。
[Abstract]:Objective to evaluate the pelvic floor function after selective cesarean section by transperineal three-dimensional ultrasound. Methods Eighty-six primipara who were delivered by selective cesarean section were examined by transperineal three-dimensional ultrasonography at 42 days postpartum. The pelvic floor tissue was observed in resting state and Valsalva, and the bladder neck movement was measured and calculated. Area of fissure of levator ani muscle, etc. According to the results of ultrasonic examination, the patients were divided into two groups: the group with pelvic floor dysfunction (observation group) and the group without pelvic floor dysfunction (control group). Results there were 18 cases of pelvic floor dysfunction (observation group) and 68 cases of no pelvic floor dysfunction (control group) in 86 cases of elective cesarean section. In the observation group, 6 cases of stress urinary incontinence, 1 case of vaginal anterior wall prolapse, 1 case of uterine prolapse, and 10 cases of other 10 cases were not detected abnormal. In the control group, 9 cases of stress urinary incontinence were clinically examined, while 59 cases were not abnormal. In the observation group, 13 cases of bladder neck increased, 2 cases of vaginal anterior wall prolapse, 2 cases of uterine prolapse and 1 case of rectocele by transperineal three-dimensional ultrasound. In the control group, no abnormality was detected by perineal three-dimensional ultrasound. The movement degree of bladder neck was significantly larger than that of the control group (P0.05), and the area of the sphincter fissure was significantly larger than that of the control group (P0.05) during the Valsalva movement in the observation group, and the difference was statistically significant. Conclusion Transperineal three-dimensional ultrasonography can detect mild pelvic floor dysfunction in early stage and can be used to evaluate the pelvic floor function after selective cesarean section.
