高频超声对不同大小甲状腺乳头状癌诊断价值的研究

【摘要】:目的:探讨各超声特征对不同大小PTC的诊断价值以寻找最适用的联合诊断方法。 材料与方法:回顾性分析在我院行甲状腺超声检查并经病理结果证实的124例207个甲状腺病灶的超声影像资料,按结节大小分3组:A组最大径≤0.5cm,B组最大径为0.5~1cm(>0.5cm且≤1cm),C组最大径>1cm。比较结节各超声特征(纵横比、边界、内部回声、钙化类型)诊断不同大小PTC的敏感性、特异性及符合率,并且评价各联合诊断标准诊断不同大小PTC的敏感性、特异性、误诊率及漏诊率;构建ROC曲线,比较纵横比与其他超声特征以及各联合诊断试验诊断不同大小PTC的准确性。 结果:①在各项超声特征中,A/T>1与微钙化仅与PTC的大小有关,表现为A/T>1(P0.001)的PTC体积较小,,表现有微钙化(P=0.093)的PTC体积较大;边界模糊仅与良性结节大小有关,表现为边界模糊(P=0.014)的良性结节体积较小;低回声及III型血供与结节性质无关,仅与结节大小有关,表现为低回声(P0.001)的结节体积较小,表现为III型血供(P0.001)的结节体积较大。 ②对整体PTC而言, A/T>1和微钙化诊断的特异性(95.5%和92.8%)、PPV(94.9%和87.0%)及整体符合率(81.2%和87.0%)较高;边界模糊及低回声诊断的敏感性(98.2%和93.5%)较高。III型血供的各项指标均较低。 ③对不同大小PTC而言,A/T>1诊断PTC的敏感性(81.4%)和特异性(96.8%)在A组最高,但在C组较低。微钙化诊断PTC的特异性在C组高于A组(60.6%vs.27.9%)。低回声及边界模糊诊断A组PTC均具有较高的敏感性(95.3%和97.9%),但特异性(19.4%和29.9%)较低。 ④ROC与多变量观察值的ROC结果显示:A/T>1对A组PTC有极高的诊断准确性(AUC=0.981,P0.001),对C组PTC无诊断准确价值(AUC=0.632,P=0.118),各超声特征对于B组PTC的诊断准确性相近,A/T>1略高于其他超声特征。 ⑤联合诊断结果显示:A、B组以A/T>1联合边界模糊、低回声及微钙化三者中任意一项为联合诊断标准,其敏感性、特异性高,且误诊率、漏诊率低于其他联合诊断标准,ROC曲线下面积亦高于其他联合诊断标准,准确性水平较高。 结论:①对于≤1cm PTC(尤其对于≤0.5cm的PTC),A/T>1是最可靠的超声诊断特征。其最适用的联合诊断公式为:A/T>1+其他三项超声特征(微钙化、边界模糊、低回声)中任意一项。 ②对于最大直径>1cm的PTC,A/T>1不适宜作为主要诊断标准;其他三项超声特征(微钙化、边界模糊、低回声)亦处于中等水平,临床应用时应当综合考虑所有的超声特异性表现。其最适用的联合诊断公式为:边界模糊+其他三项超声特征中任意一项。 ③血供类型对PTC及不同大小PTC均没有明确的预测价值。
