慢性心衰患者超声心功能指标与血清Hcy的相关性

【摘要】:目的:研究血清同型半胱氨酸(Hcy)水平与慢性心力衰竭(CHF)患者超声心功能指标的相关性。方法:研究对象选取2014年4月至2015年6月间收治的CHF患者88例(心衰组),同期选择进行体检的健康者65例作为对照组,采用彩色多普勒超声对两组患者的左室射血分数(LVEF)和左室舒张末期内径(LVEDd)等心功能指标进行测定,同时测定血清Hcy和N端B型脑钠肽前体(NT-pro BNP)水平,比较两组和不同心功能分级患者的血清Hcy和NT-pro BNP水平、LVEF及LVEDd差异,分析血清Hcy和NT-pro BNP水平与LVEF、LVEDd之间的相关性,分析血清Hcy水平对心血管事件影响。结果:CHF组的Hcy、NTpro BNP水平及LVEDd均显著高于对照组(P0.01),LVEF明显低于对照组(t=31.78,P=0.00);不同心功能分级(II~IV级)的Hcy、NT-pro BNP、LVEDd及LVEF比较均具有统计学差异(P0.01);Pearson相关性分析显示,CHF患者的血清Hcy水平与LVEF呈负相关(r2=0.974,P0.01),与LVEDd呈正相关(r~2=0.896,P0.05),且血清Hcy水平越高,CHF患者的心血管事发生率越高。结论:CHF患者的血清Hcy和NTpro BNP水平较健康者明显升高,且随着心功能恶化而增加,同时与LVEF呈负相关,与LVEDd正相关,高Hcy水平还可增加发生心血管事件风险,因此Hcy水平有望作为CHF病情变化的新的诊断指标。
[Abstract]:Objective: to study the relationship between serum homocysteine (Hcy) level and echocardiographic cardiac function in patients with chronic heart failure (CHF). Methods: 88 patients with CHF (heart failure group) admitted from April 2014 to June 2015 and 65 healthy persons who underwent physical examination were selected as control group. Left ventricular ejection fraction (LVEF) and left ventricular end-diastolic diameter (LVEDd) were measured by color Doppler ultrasound. Serum Hcy and N-terminal B-type brain natriuretic peptide precursor (NT-pro BNP) were measured. The levels of serum Hcy and NT-pro BNP, LVEF and LVEDd were compared between the two groups and patients with different cardiac function grade. The correlation between serum Hcy and NT-pro BNP levels and LVEF,LVEDd was analyzed, and the influence of serum Hcy level on cardiovascular events was analyzed. Results: the levels of Hcy,NTpro BNP and LVEDd in CHF group were significantly higher than those in control group (P0.01), LVEF was significantly lower than that in control group (t 31.78). The difference of Hcy,NT-pro BNP,LVEDd and LVEF in different cardiac function grade (II~IV grade) was statistically significant (P0.01). Pearson correlation analysis showed that the level of serum Hcy in CHF patients was negatively correlated with LVEF (r2n0.974, P0.01), and positively correlated with LVEDd (P 0.05). The higher the level of serum Hcy, the higher the incidence of cardiovascular events in CHF patients. Conclusion: the serum Hcy and NTpro BNP levels in patients with CHF were significantly higher than those in healthy controls, and increased with the deterioration of cardiac function, and were negatively correlated with LVEF and positively correlated with LVEDd. High Hcy levels also increased the risk of cardiovascular events. Therefore, the level of Hcy is expected to be a new diagnostic index for the changes of CHF.
