超声引导下聚桂醇和平阳霉素治疗静脉畸形效果比较

【摘要】:目的比较超声引导下聚桂醇和平阳霉素介入治疗静脉畸形的效果。方法回顾性分析我院介入治疗的53例静脉畸形患者的临床资料,按治疗方法不同分为平阳霉素组27例与聚桂醇组26例。在实时超声引导下,单用平阳霉素或聚桂醇注射治疗。4周注射1次,4次为1个疗程。治疗前超声评估畸形静脉腔的体积;疗程结束后6个月观察临床疗效及超声评估畸形静脉腔的体积缩小情况。观察术后近远期并发症,如肺栓塞、皮肤溃疡、局部肿胀、发热等。结果 53例患者均完成1个疗程治疗,并于疗程结束后6个月随访1次。治疗前,2组畸形静脉腔的体积差异无统计学意义(P0.05);治疗后,聚桂醇组畸形静脉腔的体积小于平阳霉素组,差异有统计学意义(P0.01);2组治疗后畸形静脉腔的体积均较治疗前明显缩小,差异有统计学意义(P0.01)。随访6个月时,平阳霉素组治愈率37.0%(10/27),聚桂醇组治愈率88.5%(23/26),差异有统计学意义(Z=3.838,P0.01)。聚桂醇组治疗均未出现肺栓塞。聚桂醇组并发症发生率为30.8%(8/26),差异无统计学意义(P0.05),平阳霉素组为18.5%(5/27),差异无统计学意义(2χ=0.536,P0.05)。结论对于畸形腔隙大、位置深在、回流速度快的静脉畸形,超声引导下聚桂醇泡沫硬化治疗效果优于平阳霉素,且两者并发症发生率无差异。
[Abstract]:Objective to compare the effect of interventional treatment of venous malformation with polycinolol and pingyangmycin under ultrasound guidance. Methods the clinical data of 53 patients with venous malformation treated by interventional therapy in our hospital were retrospectively analyzed. According to different treatment methods, 27 cases were divided into Pingyangmycin group (n = 27) and polycinnamol group (n = 26). Under the guidance of real-time ultrasound, pingyangmycin or polycinnamol were injected only once in 4 weeks and 4 times as a course of treatment. The volume of anomalous venous cavity was evaluated by ultrasound before treatment, the clinical effect was observed 6 months after treatment and the volume reduction of deformed venous cavity was evaluated by ultrasound. To observe the postoperative complications, such as pulmonary embolism, skin ulcer, local swelling, fever and so on. Results all 53 patients completed a course of treatment and were followed up 6 months after the end of the course. Before treatment, there was no significant difference in volume of malformed venous cavity between the two groups (P0.05); after treatment, the volume of venous cavity in polycinnamol group was smaller than that in pingyangmycin group (P0.01). After treatment, the volume of venous cavity in both groups was significantly smaller than that before treatment (P0.01). The cure rate was 37.0% (10 / 27) in Pingyangmycin group and 88.5% (23 / 26) in polycinnamol group (P 0.01). No pulmonary embolism was found in the polycinnamol group. The incidence of complications in polycinnamol group was 30.8% (8 / 26), the difference was not statistically significant (P0.05), and that in Pingyangmycin group was 18.5% (5 / 27), and there was no significant difference (蠂 ~ 2 = 0.536). Conclusion for venous malformation with large lacunae, deep position and fast reflux rate, the effect of polycinnamol foam sclerosis under ultrasound guidance is better than that of pingyangmycin, and there is no difference in the incidence of complications between the two groups.
