应用三维斑点追踪技术评价肥厚型心肌病患者左心室面积应变

【摘要】:肥厚型心肌病(hypertrophycardiomyopathyHCM)是一种常见的常染色体遗传疾病,该病病程发展缓慢,一旦逐步恶化,可危及生命,临床不良后果主要表现为心衰、猝死、及房颤导致的栓塞等,其中肥厚型非梗阻性心肌病患者,尤其是室间隔明显增厚时,在过量运动或者心率≥120次/分伴有阵发性室性心动过速时易诱发左室流出道梗阻,使得心衰及猝死的风险性增加。传统的超声心动图可以对HCM患者心室壁的厚度进行准确的测量、评估室壁运动及瓣膜功能,为临床诊断做出及时预警,但因其受到多方面的局限性,对早期心功能改变的敏感性低。近年来发展的三维斑点追踪成像技术(3D-STE)可以对左室进行实时三维全容积数据采集,并且对心肌内斑点运动进行实时追踪,评估心内膜面积形变,对左心室收缩功能的评估起着重要的作用,为临床提供较为全面的左室局部及整体的收缩功能的信息。 目的:应用超声3D-STE技术评价肥厚型心肌病(HCM)的左心室局部和整体的心内膜的面积应变,,准确的评估HCM患者心功能改变,为临床提供有价值的参考信息。 资料与方法:选取左室射血分数正常的HNCM患者28例,正常对照组30例,受检者左侧卧位,平静呼吸,同步记录心电图,应用ToshibaArtida多普勒超声仪及PST-25SX探头,经胸进行心尖四腔心切面图像采集,进行左心室全容积三维动态图像存储,获取重组后的心尖四腔心,心尖两腔心及左心室短轴基底段、中间段、心尖段图像并进行动态储存,应用机器配备的3D-STI分析软件进行脱机分析,描记成功后,仪器自动显示出左室射血分数(LVEF),左室收缩末期容积(LVESV),左室舒张末期容积(LVEDV),左心室16节段面积应变峰值(PAS)及整体的面积应变峰值(PAS-glo)参数及相应曲线图。 结果:1HNCM组和对照组相比基底段水平、乳头肌水平、心尖水平PAS明显减低,差异有统计学意义(P<0.05)。2HNCM组与对照组相比PAS-glo减低,两组之间差异有统计学意义(P<0.05)。3HNCM组与对照组相比左心室收缩末期各节段间PAS差异有统计学意义(P<0.05),其中BA、ML、MA、MI、AA、AI、AL的PAS减低为著.4PAS-glo与LVEF之间有良好的相关性(r=0.832). 结论:3D-STE可以有效评估HNCM患者左心室局部及整体的面积应变峰值,为临床提供了有效评估左室收缩功能的新方法。
