乳腺肿瘤超声造影无灌注区的病理基础探讨

【摘要】：目的:探讨乳腺肿瘤超声造影无灌注区的临床意义及其发生的病理基础。方法:回顾性分析120例乳腺肿瘤的超声造影动态图像,仔细观察这些图像中是否出现无灌注区,并进一步分析无灌注区的面积和部位等相应参数,研究其与肿块大小及病理结果中出现的特征性表现(坏死、出血、钙化和黏液样变)的相关性。结果:120例乳腺肿瘤超声造影图像中,93例出现无灌注区,在良恶性之间存在统计学差异(P=0.000,r=0.442),出现无灌注区与坏死之间有统计学差异(P=0.000,r=0.336)。但是无灌注区面积及部位与肿瘤大小,肿瘤良恶性及各种病理改变之间均无统计学意义(P0.05)。结论:超声造影无灌注区的出现有助于乳腺癌的诊断、坏死等病理变化为其提供病理基础。有关无灌注区发生发展深入的病理基础尚有待于进一步的大样本研究。
