MR三维序列在膝关节软骨成像中的对比研究及临床应用

目的:通过比较膝关节软骨信噪比(SNR)和对比噪声比(CNR),分析MR三维序列显示关节软骨的效果;以关节镜(或手术)结果为金标准,比较各序列诊断膝关节软骨病变的能力,探讨关节软骨成像的最佳序列。方法:选取16名健康志愿者(膝关节无外伤史、无任何临床症状,查体阴性,男、女各8例,年龄8-32岁,平均年龄22.25±7.58岁),常规MR扫描后行三个三维序列矢状位扫描,即:3D-FFE-SPIR、3D-FFE-WATS以及3D-Balance-FFE序列。分别测量上述各序列关节软骨、骨髓、滑液、肌肉以及背景噪声信号强度,依据公式计算相应组织的信噪比(SNR)和对比噪声比CNR)。另外,对临床可疑膝关节损伤的病人117例(男43例,女性74例,年龄25~72岁,平均49.26±6.45岁,左膝56个,右膝61个)行常规MR序列和3D-FFE-SPIR、3D-FFE-WATS以及3D-Balance-FFE序列检查,择取其中52个于检查后一周内做了关节镜(或者手术)检查与治疗的膝关节作为研究对象。采用改良式Noyes分级标准记录MR各成像序列和膝关节镜(或手术)显示各区域(髌骨关节面、股骨滑...
