SIS和聚丙烯网片植入大鼠腹壁后局部组织形态的中长期观察

【摘要】:目的:观察SIS和聚丙烯网片植入大鼠腹壁后局部组织形态中长期的改变和区别。方法:60只成年SD大鼠随机分为3组(每组20只),在大鼠腹壁形成2cm×2cm人工缺损,将聚丙烯合成网片(A组)、SIS生物网片(B组)植入大鼠腹壁后,与无网片植入组(C组)对比。术后3、9、15、21月,观察网片植入局部宏观和微观(电镜下)的形态学的改变。结果:术后SD大鼠减损例数:A组4例,B组3例,C组3例,共10例,三组间无差异(P0.05)。随着时间延长,术后各组均有不同程度的修复。聚丙烯网片组增生明显,但异物感最强,腹壁僵硬,触摸时有明显的沙粒感;SIS网片组柔软、舒适,但有较为明显的薄弱感;对照组缝合处与周围组织触感相同,偶及疤痕或结节。电镜下:聚丙烯网片组成纤维细胞增殖更为活跃,数量也较多,组织较为紧密,胶原纤维增生多且致密,胶原纤维直径粗,但异物界限稍有模糊,多层细胞管壁的较大血管,管腔逐渐增大,管壁逐渐增厚;SIS生物网片组较为稀疏,散乱,细胞数较少,胶原纤维增生效果最差,断裂模糊,组织内异物界限逐渐模糊、质地变薄变淡,有消失趋势,血管增生最少,管壁最薄,管腔也最小,周围组织较少;对照组成纤维细胞增生活跃,胶原纤维致密,血管增生较多,未见明显异物。结论:聚丙烯网片组组织增生明显,但异物感强;SIS网片组织相容性良好,但增生不佳,提供张力的结缔组织较为薄弱。开发同时具有各自优点的网片将是继续研究和应用盆底生物网片的方向。
[Abstract]:Objective: to observe the long-term changes and differences of local histomorphology after SIS and polypropylene mesh implantation into the abdominal wall of rats. Methods: sixty adult Sprague-Dawley rats were randomly divided into 3 groups (20 rats in each group) with 2 cm 脳 2 cm defect in the abdominal wall. The abdominal wall of rats in group A (group A) and group C (group C) were implanted with biofilm in group A (P < 0.05), and compared with group C (group C). Results: there were 4 cases in group A and 3 cases in group B, 3 cases in group C, 3 cases in group C, and 10 cases in group C. there was no difference between the three groups (P 0.05), and there was no difference between the three groups (P 0.05). The proliferation of polypropylene mesh group was obvious, but the foreign body feeling was the strongest, the abdominal wall was stiff, and there were obvious sand particles in SIS mesh group when touching, which were soft and comfortable, but had obvious weak feeling. The suture of the control group was the same as that of the surrounding tissue. Under the electron microscope, the proliferation of the fibrous cells was more active, the number of the fibrous cells was more, the tissue was more compact, the collagen fibers were proliferating and dense. The diameter of collagen fiber was coarse, but the boundary of foreign body was slightly blurred, the larger vessel of multilayer cell wall, the lumen gradually enlarged, the wall thickened gradually and the SIS biological mesh group was sparse, scattered, the number of cells was less, and the effect of collagen fiber proliferation was the worst. In the control group, the fibroblasts proliferate actively, the collagen fibers are dense, the vascular hyperplasia is the least, the wall is the thinnest, the lumen is the smallest, and the surrounding tissues are less. Conclusion: in the polypropylene mesh group, the tissue proliferation is obvious, but the tissue compatibility is good, but the proliferation is not good, but the foreign body sensation is strong, but the proliferation is not good. The connective tissue that provides tension is relatively weak. The development of meshes with respective advantages will be the direction of continuing research and application of pelvic floor biological meshes.
