COPD患者血清D-二聚体与基质金属蛋白酶-9测定的临床价值

【摘要】：目的：探讨慢性阻塞性肺疾病（COPD）患者急性加重期治疗前后血清D-二聚体（D-D）、基质金属蛋白酶-9（MMP-9）及基质金属蛋白酶抑制剂-1（TIMP-1）水平的变化及意义。方法：选取COPD急性加重期患者37例，正常对照组30例，用酶联免疫吸附（ELISA）方法检测其治疗前后血清中D-二聚体、MMP-9及TIMP-1的水平。结果：（1）COPD组治疗前血清D-二聚体、MMP-9及TIMP-1的水平均高于正常对照组；差别均具有统计学意义（P＜0.01）；COPD组治疗后D-二聚体、MMP-9及TIMP-1的水平也均高于正常对照组，差别均具有统计学意义（P＜0.05），尤其是TIMP-1差异更为显著（P㩳0.01）；COPD组治疗后D-二聚体、MMP-9及TIMP-1的水平明显低于治疗前，差异均有统计学意义（P㩳0.05）。（2）COPD组治疗前后，D-二聚体与MMP-9、D-二聚体与TIMP-1及MMP-9与TIMP-1均具有良好的正相关性，，且治疗前的相关系数大于治疗后。结论：（1）COPD急性加重期患者血清D-二聚体、MMP-9及TIMP-1的浓度水平增高，体内可能存在高凝状态或血栓前状态，治疗后有一定程度改善；（2）COPD患者中D-二聚体与MMP-9有良好的正相关性，二者可作为COPD患者的疾病急时相反应因子；TIMP-1与MMP-9在治疗前后水平变化的一致程度有一定差异，影响二者表达水平的因素复杂多样，在COPD急性加重期患者中二者的失衡表现尤为突出。
[Abstract]:Objective: to investigate the changes and significance of serum D dimer D-DG, matrix metalloproteinase-9 (MMP-9) and matrix metalloproteinase inhibitor (MMP-1) in patients with chronic obstructive pulmonary disease (COPD) during acute exacerbation. Methods: 37 patients with acute exacerbation of COPD and 30 normal controls were selected. The serum levels of MMP-9 and TIMP-1 were detected by Elisa before and after treatment. Results the serum levels of MMP-9 and TIMP-1 in TIMP-1 group were higher than those in normal control group before and after treatment. The difference was statistically significant (P < 0.01). The levels of MMP-9 and TIMP-1 in the TIMP-1 group were higher than those in the normal control group (P < 0.05), especially in the TIMP-1 group. After treatment, the levels of MMP-9 and TIMP-1 were significantly lower in the 0. 01 TIMP-1 group than before treatment, and the difference was statistically significant (P < 0. 01). There were good positive correlations between D-dimer and MMP-9 D- dimer and TIMP-1, MMP-9 and TIMP-1 before and after treatment, and the correlation coefficient before and after treatment was higher than that before and after treatment. Conclusion the serum levels of MMP-9 and TIMP-1 in patients with acute exacerbation of TIMP-1 may be hypercoagulable or prethrombotic, and there is a good positive correlation between D- dimer and MMP-9 after treatment. The level of TIMP-1 and MMP-9 in patients with COPD before and after treatment were different, and the factors affecting their expression were complex and varied. In patients with acute exacerbation of COPD, the imbalance between the two is particularly prominent.
