支气管肺泡灌洗治疗肺脓肿和支气管扩张合并感染的临床研究

【摘要】：目的探讨肺脓肿和支气管扩张合并感染患者行支气管肺泡灌洗疗法的临床效果研究,为临床治疗提供参考。方法选取医院2013年1-11月支气管扩张合并感染患者104例,按治疗方法的不同分为治疗组和对照组,每组各52例,所有患者均进行祛痰药及抗菌药物等常规治疗,治疗组在常规治疗基础上实施支气管肺泡灌洗疗法,对照组进行体位引流排痰法,比较两组患者的氧合指数(PaO2/FiO2)、动脉血氧分压(PaO2)、血氧饱和度(SaO2)等指标变化、治疗过程中并发症的发生及临床治疗效果,数据采用SPSS17.0软件进行统计分析。结果治疗前治疗组的PaO2/FiO2、PaO2、SaO2指标与对照组比较,差异均无统计学意义;治疗后治疗组的PaO2/FiO2为324.00±27.00显著高于对照组277.00±25.00,治疗组的PaO2(88.04±15.37)mm Hg显著高于对照组(70.47±11.23)mm Hg,治疗组的SaO2(93.41±5.53)%显著高于对照组(87.21±5.09)%,差异均有统计学意义(P0.05);治疗组并发症发生率5.8%,显著低于对照组的19.2%,差异有统计学意义(χ2=4.31,P0.05);治疗组总有效率82.7%,显著高于对照组的55.8%,差异有统计学意义(χ2=5.34,P0.05)。结论肺脓肿和支气管扩张合并感染患者行纤维支气管镜支气管肺泡灌洗疗法,临床效果确切,安全可靠,值得临床推广。
[Abstract]:Objective to investigate the clinical effect of bronchoalveolar lavage in patients with pulmonary abscess and bronchiectasis. Methods 104 patients with bronchiectasis complicated with infection in hospital from January to November 2013 were divided into treatment group and control group according to different treatment methods, 52 cases in each group. All patients were treated with expectorant drugs and antimicrobial agents. The treatment group was treated with bronchoalveolar lavage on the basis of routine treatment, and the control group was treated with body position drainage and sputum drainage. The changes of oxygen index (PaO2/FiO2), arterial partial pressure of oxygen (PaO2) and blood oxygen saturation (SaO2) were compared between the two groups. The occurrence of complications and the effect of clinical treatment were analyzed by SPSS17.0 software. Results there was no significant difference in PaO2/FiO2,PaO2,SaO2 between the treatment group and the control group before treatment. After treatment, the PaO2/FiO2 of the treatment group was 324.00 卤27.00 significantly higher than that of the control group (277.00 卤25.00), and the PaO2 of the treatment group (88.04 卤15.37) mm Hg) was significantly higher than that of the control group (70.47 卤11.23) mm Hg,). SaO2 in the treatment group (93.41 卤5.53)% was significantly higher than that in the control group (87.21 卤5.09)%, the difference was statistically significant (P0.05). The incidence of complications in the treatment group was 5.8, which was significantly lower than that in the control group (19.2g / kg, P < 0.05). The total effective rate of the treatment group was 82.7%, which was significantly higher than that of the control group (55.8%). The difference was statistically significant (蠂 2 + 5.34% P 0.05). Conclusion bronchoalveolar lavage therapy for patients with pulmonary abscess and bronchiectasis complicated with infection is effective, safe and reliable.
