暴力犯罪服刑人员自杀意念的相关心理因素研究

【摘要】:目的1.了解暴力犯罪服刑人员中自杀意念、抑郁、焦虑、冲动性和童年期创伤的发生率,以及户籍、就业、烟酒史和毒品使用、家庭信息和自杀行为等情况。2.探讨自杀意念与抑郁、焦虑、冲动性和童年期创伤的关系,以及有自杀意念组与无自杀意念组在抑郁、焦虑、冲动性和童年期创伤上的差异。3.建立多元线性回归方程,了解抑郁、焦虑、冲动性和童年期创伤对自杀意念的预测作用。方法1.本研究采用方便取样,被试为江苏省某监狱的2088名男性服刑人员,选取其中因暴力犯罪入狱的服刑人员共451人。排除严重躯体疾病和其他精神障碍者,排除SIOSS掩饰因子≥4分,共入组399人。有自杀意念组:SIOSS量表总分≥12分作为阳性筛查标准,入组67人;无自杀意念组:SIOSS量表总分12分,入组332人。2.全部服刑人员填写一般情况问卷、自杀意念量表(SIOSS)、广泛性焦虑量表(GAD-7)、中文版患者健康问卷-抑郁症状群量表(PHQ-9)、Barratt冲动性量表第十一版(BIS-11)和童年期创伤量表(CTQ-SF)。3.采用SPSS22.0进行统计分析,根据数据的类型应用描述性分析、正态性检验、卡方检验、独立样本t检验、皮尔逊相关分析及多元线性逐步回归分析。所有检验均以P0.05为差异有统计学意义。结果1.暴力罪犯服刑人员自杀意念和自杀未遂的检出率为16.79%(67/399)和3.26%(13/399)。按照焦虑、抑郁和童年期创伤划界值分组后,有无自杀意念二组差异有统计学意义(P0.01)。有无自杀意念二组在户籍来源、就业情况、吸烟和饮酒上无统计学差异,在吸毒情况上接近统计学差异(P=0.059)。有无自杀意念二组在父亲和母亲养育方式上差异有统计学意义(分别为P0.01,P0.05)。2.有自杀意念组在抑郁、焦虑、冲动性总分及其维度(注意力冲动性、运动性冲动性和无计划冲动性)和童年期创伤总分及其分量表(情感虐待、躯体虐待、情感忽视和躯体忽视)上的得分均高于无自杀意念组,差异有统计学意义(均P0.01),二组在性虐待上无统计学差异。3.除睡眠因子与躯体虐待和性虐待相关无统计学意义,自杀意念与抑郁、焦虑、冲动性和童年期创伤及其因素之间呈显著的两两正相关(r=0.091~0.914,P0.05)。4.仅考虑各量表总分建立多元线性回归模型,抑郁、童年期创伤、焦虑、冲动性总分均对于自杀意念有正向预测作用(R2=0.476,自杀意念总分=-1.727+0.322×抑郁总分+0.051×童年期创伤总分+0.184×焦虑总分+0.053×冲动性总分),且能够解释47.6%的变异量(P0.01)。5.考虑各量表及其分量表的分值建立多元线性回归模型,抑郁总分、情感忽视、焦虑总分、无计划冲动性因子和情感虐待对于自杀意念有正向预测作用,躯体虐待对自杀意念有负向预测作用(R2=0.506,自杀意念总分=-1.88+0.344×抑郁总分+0.155×情感忽视+0.181×焦虑总分+0.11 ×无计划冲动性+0.216×情感虐待-0.125×躯体虐待),且能够解释50.6%的变异量(P0.01)。结论1.暴力犯罪服刑人员的自杀意念、抑郁、焦虑、高冲动性和童年期虐待问题较为常见。父亲和母亲养育方式与自杀意念有关联,有自杀意念组的服刑人员父母不良养育方式明显偏多。2.有自杀意念组服刑人员在抑郁、焦虑、冲动性和童年期创伤的情况较严重,而二组性虐待接近统计差异。自杀意念与抑郁、焦虑、冲动性和童年期创伤之间显著正相关。3.抑郁、焦虑、冲动性和童年期创伤均对自杀意念有正向预测作用。考虑分量表后,抑郁、情感忽视、焦虑、无计划冲动性因子和情感虐待对自杀意念有正向预测作用,而躯体虐待有负向预测作用。
