神经肌肉电刺激与酸性刺激治疗急性脑卒中吞咽障碍口腔期的效果

【摘要】：目的观察急性/亚急性脑卒中吞咽障碍口腔期患者经神经肌肉电刺激(NMES)、酸性刺激治疗前后功能性经口摄食量表(FOIS)变化及不良反应发生情况。方法 2013年1月至2015年6月,本科90例急性/亚急性脑卒中吞咽障碍口腔期患者随机分为NMES组(n=30)、酸性刺激组(n=30)和综合组(n=30)。各组在常规吞咽功能训练基础上分别应用NMES疗法、酸性刺激疗法、NMES结合酸性刺激疗法。分别于治疗3周、12周采用口腔功能评分方法,观察患者FOIS变化情况。结果治疗3周后,综合组FOIS变化明显优于酸性刺激组(P0.01);治疗12周后,综合组FOIS变化均优于电刺激组和酸性刺激组(P0.05)。研究期间无任何治疗相关不良事件发生。结论急性或亚急性脑卒中吞咽障碍口腔期的患者,早期应用NMES结合酸性刺激治疗,有利于提高患者口腔控制食物能力、协调吞咽动作,防止并发症发生。
[Abstract]:Objective to observe the changes of functional oral intake scale (FOIS) and adverse reactions in patients with oral dysphagia after acute / subacute stroke treated by neuromuscular electrical stimulation and acid stimulation. Methods from January 2013 to June 2015, patients with dysphagia were enrolled in this study. 90 patients with acute / subacute stroke at oral stage of dysphagia were randomly divided into NMES group (n = 30), acid stimulation group (n = 30) and comprehensive group (n = 30). Each group was treated with NMES on the basis of routine swallowing function training. After 3 weeks of treatment, the changes of FOIS in patients were observed. Results after 3 weeks of treatment, the changes of FOIS in the combined group were significantly better than those in the acid stimulation group (P 0.01), and after 12 weeks of treatment, the changes of FOIS in the combined group were significantly better than those in the acid stimulation group, and 12 weeks after treatment, the changes of FOIS in the combined group were significantly better than those in the acid stimulation group. The changes of FOIS in the combined group were better than those in the electrical stimulation group and the acid stimulation group (P 0.05). There were no adverse events associated with treatment during the study. Conclusion the patients with acute or subacute stroke with oral dysphagia were treated with NMES combined with acid stimulation at early stage. It can improve oral food control ability, coordinate swallowing and prevent complications.
