卒中后患者急性期单侧空间忽略的临床研究

【摘要】：目的探索卒中患者急性期单侧空间忽略症的发病率及其与病变部位、性质的关系。方法所有入组患者接受苹果删除试验筛查忽略症,区分忽略症的两种形式,并比较以下3组人群中忽略症的发病率:(1)出血性卒中患者与缺血性卒中患者;(2)左侧大脑半球病变患者与右侧大脑半球病变患者;(3)大脑半球病变患者与小脑、脑干病变患者。结果本研究入组169例卒中后患者,筛查出卒中后单侧空间忽略症患者34例(20.1%),包括19例以自我为中心的忽略症,22例非自我为中心的忽略症,相关分析提示两种形式的忽略症之间无明显相关。忽略症的发病率在缺血性卒中组和出血性卒中组、左侧大脑半球和右侧大脑半球病变组的比较差异均无统计学意义。而大脑半球病变患者忽略症发病率则高于脑干、小脑病变患者。结论卒中后患者忽略症发病率不低,两种形式的忽略症没有明显相关。忽略症常见于大脑半球病变患者,但左右侧半球病变没有差别,而病变性质对忽略症发病没有影响。
[Abstract]:Objective to explore the incidence of unilateral spatial neglect in acute stroke patients and its relationship with the location and nature of lesion. Methods all patients were screened by apple deletion test to distinguish the two forms of neglect. The incidence of neglect was compared in the following three groups:% 1) hemorrhagic stroke and ischemic stroke 2) left hemisphere lesion and right hemisphere lesion. Results among 169 patients with stroke, 34 patients with unilateral spatial neglect after stroke were screened out, including 19 patients with self-centered neglect and 22 patients with non-self-centered neglect. Correlation analysis showed that there was no significant correlation between the two forms of neglect. The incidence of neglect was found in ischemic stroke and hemorrhagic stroke. There was no significant difference between the left cerebral hemisphere and the right cerebral hemisphere, while the incidence of neglect was higher in the patients with cerebral hemisphere disease than in the brainstem and cerebellar lesion. Conclusion the incidence of neglect is not low in the patients with stroke. There is no significant correlation between the two forms of neglect, which are common in patients with cerebral hemispheric lesions, but there is no difference between the left and right hemispheric lesions, and the nature of the lesions has no effect on the pathogenesis of the disease.
