三黄烧伤灵对休克期大鼠内脏功能保护作用的实验研究

【摘要】：目的探索外用中药制剂对严重烫伤休克期大鼠内脏的保护作用及机制。方法通过血液检测、组织病理学观察、免疫组化、ELISA等方法在休克期的8h、24h、48h测定小肠粘膜HSP70；肺组织丙二醛含量、还原型谷胱甘肽含量、肺质量(湿)/肺质量(干)比；肾功能测定(Bun、Cr)、肾组织髓过氧化物酶(MPO)测定；血清中的TNF-α、IL-6。结果三黄烧伤灵治疗组各时间点HPS70表达均较碘伏治疗组明显升高,烫伤早期Wistar大鼠小肠粘膜组织损伤明显,三黄烧伤灵Wistar治疗组大鼠小肠粘膜组织损伤较碘伏治疗组组明显减轻；三黄烧伤灵治疗组Wistar大鼠肺脏中丙二醛含量及还原型谷胱甘肽含量组显著高于碘伏治疗组,肺质量(湿)/肺质量(干)比显著低于碘伏治疗组；三黄烧伤灵治疗组血浆BUN和Cr与碘伏治疗组比较有明显下降,局部髓过氧化物酶活性降低；三黄烧伤灵治疗组Wistar大鼠伤后48hTNF-α、IL-6均明显低于碘伏治疗组。结论三黄烧伤灵、膏状,做为一种活血化瘀、清热解毒之中药,可有效的阻止细菌的侵入,可减轻创面疼痛,是因为其可以有效的阻碍空气对神经末梢的刺激；同时还可以改善创面微循环、并可促进创面生长。通过组织吸收后改变内毒素的结构,以灭活解毒或降低其毒素。通过减少内毒素的释放,促进内毒素的排泄,减弱或消除TNF-α、IL-6的作用,以达到保护内脏的目的。
[Abstract]:Objective to explore the protective effect and mechanism of external Chinese medicine preparation on visceral organs in rats with severe scald shock.Methods HSP70 of small intestine mucosa was determined by blood examination, histopathological observation, immunohistochemistry and Elisa at 8 h, 24 h and 48 h after shock, and the contents of malondialdehyde (MDA), glutathione (GSH) and lung mass (wet / dry) were measured in lung tissue.The determination of renal function and myeloperoxidase (MPO) in renal tissue and TNF- 伪 and IL-6 in serum.Results the expression of HPS70 in Sanhuangzangling group was significantly higher than that in iodophor group at each time point, and the mucosal injury of small intestine was obvious in Wistar rats at the early stage of scalding.The intestinal mucosal tissue damage in Sanhuangzangling Wistar group was significantly less than that in iodophor group, and the content of malondialdehyde and glutathione in lung of Wistar rats in Sanhuangzangling group was significantly higher than that in iodophor group.The lung mass (wet / dry) ratio was significantly lower than that in iodophor group, the plasma BUN and Cr in Sanhuang Zangling group were significantly lower than those in iodophor group, and the local myeloperoxidase activity was lower in Sanhuangzangling group than that in iodophor group.The levels of TNF- 伪 IL-6 in Sanhuang Burn Ling group were significantly lower than those in iodophor group at 48 h after injury.Conclusion Sanhuang Burn Ling, as a traditional Chinese medicine of activating blood circulation, removing blood stasis, clearing heat and detoxifying, can effectively prevent bacterial invasion and relieve wound pain, because it can effectively block the stimulation of air to nerve endings.It can also improve wound microcirculation and promote wound growth.Change the structure of endotoxin by tissue absorption to inactivate or detoxify it.By reducing the release of endotoxin, promoting the excretion of endotoxin and attenuating or eliminating the effect of TNF- 伪 and IL-6, the visceral protection can be achieved.
