应用磁共振弥散张量成像观察急性脑梗死患者神经纤维束与肢体肌力恢复的相关性

【摘要】:目的探讨磁共振弥散张量成像(DTI)在急性脑梗死患者急性期测得的各异系数(FA)值、表现弥散系数(ADC)值与肢体肌力评分的相关性,旨在分析DTI评价肢体肌力降低的临床价值。方法收集该院2015年6月至2016年6月诊断为急性脑梗死并接受治疗的患者20例,对所有患者在发病3d内测定简化Fugl-Meyer运动功能评分并行DTI检查神经纤维束的分布情况,FA值、ADC值的变化。结果梗死侧FA值、ADC值与健侧对应处比较差异有统计学意义(t=-8.70、-18.70,P0.05);梗死侧内囊后肢FA值、ADC值与健侧对应处比较差异有统计学意义(t=-5.16、-5.08,P0.05);梗死侧内囊后肢FA值与简化Fugl-Meyer运动功能评分呈正相关(R=0.863,P=0.013)。结论急性梗死灶与内囊后肢的FA值、ADC值均比健侧正常白质的FA值、ADC值低,内囊后肢的FA值与简化Fugl-Meyer运动功能评分相关性较为紧密。
[Abstract]:Objective to investigate the difference coefficient (FA) values measured by magnetic resonance diffusion tensor imaging (DTI) in acute cerebral infarction patients, the correlation of dispersion coefficient (ADC) value and limb muscle strength score, and to analyze the clinical value of DTI to evaluate the reduction of limb muscle strength. Methods the hospital was diagnosed as acute cerebral infarction and accepted from June 2015 to June 2016. 20 patients were treated with a simplified Fugl-Meyer motor function score and DTI examination of the distribution of nerve fibers, FA value, and ADC value in all patients. Results the FA value of the infarct side was statistically significant (t=-8.70, -18.70, P0.05), ADC value and the counterpart of the contralateral side (t=-8.70, -18.70, P0.05); the FA value of the posterior limb of the internal capsule of the infarct, ADC value and the healthy side The corresponding differences were statistically significant (t=-5.16, -5.08, P0.05); the FA value of the posterior limb of the infarct side was positively correlated with the simplified Fugl-Meyer motor function score (R=0.863, P=0.013). Conclusion the FA of the acute infarct and the posterior limb of the internal capsule was lower than the FA value of the normal white matter in the healthy side, the ADC value, the FA value of the posterior limb of the inner capsule and the simplified exercise function. The correlation of score was close.
