百格斯和乳酸依沙吖啶与碘伏换药敷料的临床疗效及细胞增殖毒性比较

【摘要】:目的了解临床常用伤口抗菌敷料百格斯、乳酸依沙吖啶和碘伏对头面部感染伤口的疗效、成本以及细胞增殖毒性。方法回顾性分析2013年7月—2014年4月北京同仁医院急诊外科收治的各种原因所致头面部皮肤裂伤清创缝合术后感染患者46例,根据采用的抗菌溶液不同,将患者分为百格斯组(16例)、碘伏组(15例)及乳酸依沙吖啶组(15例),分别采用相应消毒剂进行换药。记录患者伤口愈合时间及治疗费用。将CHL(中国仓鼠成纤维肺细胞)接种于96孔板,分别稀释碘伏(0.000 5%~0.1%)、过氧化氢(0.002 5%~0.5%)、百格斯(0.1%~20%)及乳酸依沙吖啶(0.5%~100%)至8个不同浓度,加入培养基孵育6 h。用酶标仪在450 nm波长处检测各孔的吸光度(OD)值,以吸光率(%)=(OD_(待测组)/OD_(对照组))×100%评估细胞增殖抑制率。结果百格斯组、碘伏组和乳酸依沙吖啶组愈合时间、7 d愈合率及14 d愈合率比较,差异均无统计学意义(P0.05)。各组治疗费用比较,差异有统计学意义(P0.05);其中,碘伏组和乳酸依沙吖啶组治疗费用低于百格斯组,差异有统计学意义(P0.05)。孵育6 h后,细胞增殖抑制率为乳酸依沙吖啶(41.922%)百格斯(5.528%)过氧化氢(0.058%)碘伏(0.006%)。结论与碘伏和乳酸依沙吖啶比较,应用百格斯对头面部感染伤口治愈时间和治愈率均未见显著差异,但治疗费用较高,细胞增殖毒性中等。患者可根据免疫力及经济水平,选择合理消毒剂。
[Abstract]:Objective to investigate the efficacy, cost and cytotoxicity of bactericidal dressing, ethacridine lactate and iodophor in the treatment of head and face infection wounds. Methods from July 2013 to April 2014, 46 patients with head and facial skin laceration after debridement and suture were retrospectively analyzed. The patients were divided into Biggs group (n = 16) and iodophor group (n = 15) and ethacridine lactate group (n = 15). The wound healing time and treatment cost were recorded. CHL (Chinese hamster fibroblast) was inoculated on 96 well plate and diluted with 0. 5% iophor 0.000 and 0. 1%, respectively. Hydrogen peroxide 0.002 5 5 was added to culture medium for 6 h. The concentration of CHL was 0. 0025. 0. 5 and 20%) and ethacridine lactic acid was 0. 5 and 0. 5%, respectively) and incubated in culture medium for 6 h. The absorbance and ODV values of each hole were detected at 450 nm wavelength by enzyme labeling instrument, and the inhibition rate of cell proliferation was evaluated by the absorptivity of ODSP (the group to be tested) / ODSP (control group) 脳 100%. Results there was no significant difference in the healing time of 7 days and 14 days in Baiggs group, iodophor group and ethacridine lactate group. The treatment cost of each group was significantly lower than that of Biggs group (P 0.05), and the treatment cost of iodophor group and lactic acid ethacridine group was lower than that of Biggs group (P 0.05). After incubation for 6 h, the inhibition rate of cell proliferation was 41.922% ethacridine lactic acid (41.922) and (5.528%) hydrogen peroxide (0.058). Conclusion compared with iodophor and ethacridine lactate, there is no significant difference in cure time and cure rate of head and face infection wound treated with Baigs, but the cost of treatment is higher and the toxicity of cell proliferation is moderate. The patient can choose reasonable disinfectant according to immunity and economic level.
