超声测量在评价体外冲击波治疗跖腱膜炎效果中的应用

【摘要】：目的探讨超声测量在评价体外冲击波治疗跖腱膜炎效果中的应用。方法应用超声观察和测量临床诊断为跖腱膜炎的21例中老年患者(患者组)的患侧跖腱膜跟骨附着部的厚度,并测量35例正常中老年人的单侧跖腱膜相应部位作为对照(正常组)。其中,患者组于体外冲击波首次治疗前和治疗1个疗程(每周1次治疗,5次/疗程)后各测量1次,并进行VAS评分以评估疗效。结果治疗前,患者组患足的跖腱膜跟骨附着部的厚度为(0.582±0.013)cm,较正常组的厚度(0.333±0.001)cm明显增加(P0.01)。经1个疗程冲击波治疗后,患者组腱膜跟骨附着部的厚度减少为(0.517±0.013)cm,与治疗前相比差异有统计学意义(P0.01),但仍大于正常组的厚度(P0.01);且VAS评分较治疗前明显减少,治疗有效率达57.28%(P0.01)。结论超声测量用于评价体外冲击波治疗跖腱膜炎的效果确切,且兼具无创、便捷等优点。
[Abstract]:Objective to evaluate the effect of extracorporeal shock wave on plantar aponeurosis. Methods Ultrasound was used to observe and measure the thickness of calcaneal attachment of the metatarsal aponeurosis in 21 middle and aged patients (patient group) diagnosed clinically as metatarsal aponeurosis. The corresponding position of unilateral metatarsal aponeurosis was measured in 35 normal aged people as control group (normal group). The patients were measured before the first treatment of extracorporeal shock wave (ESWL) and 1 course of treatment (5 times a week), and VAS scores were used to evaluate the efficacy. Results before treatment, the thickness of calcaneal attachment of metatarsal aponeurosis in the patients group was 0.582 卤0.013 cm, which was significantly higher than that in the normal group (0.333 卤0.001)cm). After one course of shock wave treatment, the thickness of aponeurosis calcaneal attachment in the patients group decreased to 0.517 卤0.013 cm, which was significantly different from that before treatment, but still larger than that of the normal group, and the VAS score was significantly reduced compared with that before treatment, and the effective rate was 57.28% (P0.01). Conclusion Ultrasonography can be used to evaluate the effect of extracorporeal shock wave in the treatment of plantar aponeurosis.
