ERK信号通路对急性ST段抬高型心肌梗死患者急诊PCI术后冠脉无复流的影响

【摘要】:目的探讨ERK信号通路对急性ST段抬高型心肌梗死患者急诊PCI术后冠脉无复流的影响。方法选择急性ST段抬高型心肌梗死(STEMI)行急诊PCI的患者38例,根据术后TIMI分级分为无复流患者16例(无复流组)和有效灌注者22例(再灌注组);另选择20例健康体检者作为对照组。采用全自动生化分析仪测定各组血清总胆固醇(TC)甘油三酯(TG)低密度脂蛋白胆固醇(LDL-C)高密度脂蛋白胆固醇尿素氮肌酐尿酸等生化指标,利用RT-PCR方法测定各组外周血单个核细胞ERK mRNA及bad mRNA表达。结果无复流组、再灌注组血清TG、TC、LDL-C水平明显高于对照组(P均0.05);无复流组血清LDL-C水平明显高于再灌注组(P0.05)。对照组单个核细胞ERK mRNA表达水平高于无复流组及再灌注组,bad mRNA表达水平低于无复流组及再灌注组(P均0.05);无复流组ERK mRNA表达水平低于再灌注组,bad mRNA表达水平高于再灌注组(P均0.05)。单个核细胞ERK mRNA与bad mRNA表达水平呈负相关(r=-0.61,P0.05)。结果在STEMI患者急诊PCI术无复流的患者中ERK信号通路表达活性下降;ERK信号通路活性降低及bad mRNA表达增强可能参与了急诊PCI术后冠脉无复流的发生。
[Abstract]:Objective to investigate the effect of ERK signal pathway on coronary artery free reflow after emergency PCI operation in patients with acute ST elevation myocardial infarction. Methods 38 patients with acute ST segment elevation myocardial infarction (STEMI) were selected for emergency PCI. According to the postoperative TIMI classification, 16 patients without reflow (no reflow group) and 22 patients with effective perfusion (reperfusion group) were selected, and 20 were selected. The serum total cholesterol (TC), triglyceride (TG), low density lipoprotein cholesterol (LDL-C), high density lipoprotein cholesterol (HDL), urea nitrogen, creatinine and uric acid were measured by a full automatic biochemical analyzer, and the expression of ERK mRNA and bad mRNA in peripheral blood mononuclear cells in each group was measured by RT-PCR method. The serum levels of TG, TC and LDL-C in the reperfusion group were significantly higher than those in the control group (P 0.05), and the serum LDL-C level in the non reflow group was significantly higher than that in the reperfusion group (P0.05). The expression level of ERK mRNA in the mononuclear cells in the control group was higher than that without reflow group and reperfusion group. The level of bad mRNA in the control group was lower than that in the non reflow group and the reperfusion group (P 0.05); no reflow was found. The expression level of ERK mRNA was lower than that in the reperfusion group, and the expression level of bad mRNA was higher than that in the reperfusion group (P 0.05). The ERK mRNA in mononuclear cells was negatively correlated with the mRNA expression level of bad (r=-0.61, P0.05). Strong may be involved in the occurrence of coronary no reflow after emergency PCI.
