大黄碳酸氢钠对严重烧伤胃肠道动力功能保护作用的研究

【摘要】:研究目的:观察早期应用大黄碳酸氢钠片对严重烧伤胃肠道动力功能的保护作用。 方法:将54只SD大鼠随机分为3组:正常组(N组,n=6)、对照组(B组,n=24)、治疗组(S组,n=24)。后两组又分为伤后6h、12h、24h、48h四个时相点,每个时相点6只大鼠。处理方法:①N组:不做处理,观察硫酸钡混悬液在小肠中移行距离占整个小肠的百分比值以及血浆中MTL、SP、VIP、SS的含量。②B组:制备成30%背部Ⅲ度烫伤模型,并于伤后立刻将2ml生理盐水灌胃,每天2次;③S组:同样方法制备成30%背部Ⅲ度烫伤模型,并与B组同时段将大黄碳酸氢钠片1片溶解于2ml生理盐水后灌胃。各时相点前30min行大鼠硫酸钡混悬液灌胃(2mL/100g),于各时相点颈椎脱臼处死大鼠,剖腹,取胃及小肠平铺于黑纸上,观察硫酸钡混悬液在小肠中移行距离占整个小肠的百分比值;并同时抽取大鼠腹腔静脉血,用酶联免疫分析法检测血浆MTL、SP、VIP、SS的含量。 结果:(1)大鼠小肠推进的百分率在伤后6h开始下降,12h降至最低值,伤后24h开始回升,48h仍低于正常组;治疗组各时相点小肠推进的百分率明显高于对照组。(2)MTL和SP在伤后6小时开始下降且达到最低值,12小时开始上升,48小时仍低于正常值;治疗组MTL和P物质下降幅度明显降低,各时相点均高于对照组,伤后48小时基本接近正常值。(3)VIP和SS伤后6小时达到最高值,伤后12小时开始下降,伤后48小时仍高于正常值;治疗组VIP和SS上升幅度较低,且各时相点均低于对照组,伤后48小时基本恢复到正常水平。结论:1.严重烫伤早期胃肠道的蠕动及推进功能降低。 2.大黄碳酸氢钠能调控严重烫伤早期血浆中胃肠激素的分泌和释放,,改善烧伤后胃肠道动力系统抑制状态,从而保护烧伤后胃肠动力功能。
