老年急性心肌梗死合并心源性休克患者经皮冠状动脉介入术后联合应用主动脉球囊反搏对术后的影响

【摘要】：目的探讨老年急性心肌梗死(AMI)合并心源性休克(CS)患者经皮冠状动脉介入术(PCI)后联合应用主动脉球囊反搏(IABP)对术后的影响。方法选择50例老年AMI合并CS患者PCI后联合应用IABP患者作为观察组,另选50例老年AMI合并CS患者PCI术后未联合IABP的患者作对照组。比较IABP术前、术后不同时间点血流动力学参数、IABP患者并发症、住院死亡率、30 d死亡率及死因。结果 IABP联合PCI在治疗AMI合并CS时,IABP植入前与术后1 h及24 h收缩压(SBP)及舒张压(DBP)比较均有显著上升(P0.05),平均动脉压Ⅱ有统计学差异(P0.05);IABP植入术后患者心率(HR)降低(P0.05);血氧饱和度、左室射血分数比较有统计学差异(P0.05)。需要进行机械通气是预后不良特征(P0.05,OR=9.634,95%CI 1.211~70.244)。结论对老年AMI合并CS患者行PCI联合IABP可以有效改善患者血流动力学状态,但患者住院期间死亡率和30 d死亡率仍保持较高水平,机械通气是患者预后不良的特征性指标。
[Abstract]:Objective to investigate the effect of percutaneous coronary intervention (PCI) combined with aortic balloon counterpulsation (IABP) on elderly patients with acute myocardial infarction (AMI) complicated with cardiogenic shock (CS). Methods 50 elderly patients with AMI combined with CS after PCI were selected as observation group, and 50 elderly patients with AMI and CS without IABP after PCI were selected as control group.
