早期持续性血液滤过联合宣白承气汤治疗脓毒症肺损伤

【摘要】:目的研究早期持续性血液滤过联合宣白承气汤对脓毒症肺损伤的治疗效果。方法选取48例ICU初诊为脓毒症肺损伤患者,随机分为对照组和观察组,每组24例。对照组给予连续性静脉-静脉血液滤过(CVVH)治疗,观察组在对照组治疗基础上加用宣白承气汤治疗。比较两组患者的住院时间、机械通气时间、ICU滞留时间,比较两组患者治疗前后氧合指数(PaO_2/FiO_2)、平均动脉压(MAP)、血乳酸(LAC)、血肌酐(SCr)、超敏C反应蛋白(hs-CRP)、急性生理与慢性健康(APACHEⅡ)评分。结果观察组的住院时间、机械通气时间、ICU滞留时间显著短于对照组,差异有统计学意义(P0.01)。两组治疗后的PaO_2/FiO_2、MAP、LAC、SCr、hs-CRP水平及APACHEⅡ评分等指标均显著低于治疗前,差异有统计学意义(P0.05)。治疗后,观察组的PaO_2/FiO_2、MAP、LAC、SCr、hs-CRP水平及APACHEⅡ评分显著低于对照组,差异有统计学意义(P0.05)。结论在CVVH治疗的基础上加用宣白承气汤治疗脓毒症肺损伤患者,可进一步缩短患者的治疗时间,进一步改善患者的急性生理功能和慢性健康状态,有着更为显著的临床效果。
