右美托咪定对脓毒症大鼠急性肾损伤的保护作用及对肾组织HMGB1mRNA表达的影响

【摘要】:【目的】观察右美托咪定(Dexmedetomidine,DEX)对脓毒症大鼠急性肾损伤(AKI)的保护作用及对肾组织高迁移率族蛋白1(HMGBl)mRNA表达及血清胱抑素C(Cys C)水平的影响,探讨DEX对脓毒症大鼠AKI保护作用的相关机制,为其应用于脓毒症临床治疗提供理论基础。 【方法】48只健康成年雄性SD大鼠,体重240-270g,采用随机数表法分为3组(n=16):假手术组(SHAM组)、模型组(CLP组)、右美托咪定组(DEX组)。CLP组和DEX组采用盲肠结扎与穿孔术(CLP)制备大鼠脓毒症诱发AKI模型。术后1h,DEX组腹腔内注射DEX40μg/kg,SHAM组和CLP组给予等容量生理盐水。各组分别于术后24h、48h时取血后随机处死8只大鼠,留取双侧肾脏。分别应用免疫比浊法检测血清Cys C水平,实时荧光定量PCR法检测肾组织HMGB1mRNA表达水平,,光学显微镜观察肾组织病理学损伤情况。 【结果】与SHAM组同时间点比较,CLP组和DEX组术后24h、48h肾组织HMGB1mRNA表达明显上调,血清Cys C水平显著升高(P0.05),肾组织均有不同程度的病理学损伤;与CLP组同时间点比较,DEX组术后24h、48h肾组织HMGB1mRNA表达下调,血清Cys C水平降低(P0.05),肾组织病理学损伤减轻。 【结论】DEX能有效减轻脓毒症大鼠肾组织病理损伤,降低血清Cys C、肾组织HMGB1mRNA表达水平,对脓毒症急性肾损伤具有一定保护作用,其机制可能与DEX抑制肾组织HMGB1mRNA的表达有关。
[Abstract]:[objective] to observe the protective effect of right metomidate (Dexmedetomidine,DEX) on (AKI) and the expression of high mobility group protein 1 (HMGBl) mRNA in renal tissue and the level of serum cystatin C (Cys C) in rats with acute renal injury induced by sepsis. To explore the protective mechanism of DEX on AKI in septic rats, and to provide a theoretical basis for its application in the clinical treatment of sepsis. [methods] 48 healthy adult male SD rats weighing 240? 270 g were randomly divided into 3 groups (n = 16): sham operation group (SHAM group) and model group (CLP group). The model of sepsis induced by AKI in DEX group (). CLP group) and DEX group (DEX group) was induced by cecal ligation and puncture with (CLP). One hour after operation, DEX 40 渭 g / kg,SHAM and CLP were injected intraperitoneally into the DEX group. At 24 h and 48 h after operation, 8 rats in each group were killed at random and bilateral kidneys were retained. Serum Cys-C level was measured by immunoturbidimetric method, HMGB1mRNA expression was detected by real-time fluorescence quantitative PCR, and pathological damage of renal tissue was observed by optical microscope. [results] compared with SHAM group, the expression of HMGB1mRNA and serum Cys-C were up-regulated and serum Cys-C level was significantly increased in CLP group and DEX group 24 h after operation (P0.05), and there were different degrees of pathological injury in renal tissue. Compared with CLP group, the expression of HMGB1mRNA in renal tissue and serum Cys C level were decreased in DEX group at 24 h and 48 h after operation (P0.05), and the renal histopathological injury was alleviated. [conclusion] DEX can effectively reduce the pathological injury of renal tissue, decrease the expression level of serum Cys-C and HMGB1mRNA in renal tissue of sepsis rats, and has a protective effect on acute renal injury of sepsis. The mechanism may be related to the inhibition of HMGB1mRNA expression in renal tissue by DEX.
