糖尿病酮症酸中毒患者的实验指标与病情相关性分析

【摘要】:目的探讨糖尿病酮症酸中毒(DKA)和高血糖高渗状态(HHS)患者相关的实验室检查与病情的关系。 方法以同济医院1998.01-2010.04收治入院的221位糖尿病酮症酸中毒患者(包括糖尿病酮症酸中毒并高血糖高渗状态)为研究对象。根据动脉血气分析测定结果分为轻、中、重度三组。回顾性分析221例糖尿病因急性并发症入院患者的临床和实验资料。 结果实验室指标经统计学t检验显示,在DKA和HHS时,,血白细胞计数、血钾、血钠、血肌酐、血淀粉酶、肌酶谱与病情轻重度相关,(P<0.05),差异均有统计学意义;红细胞计数、红细胞压积、血小板计数、血钙、血镁及胆固醇无明显差别。 结论在DKA和HHS中除血气分析是判断病情轻重的指标外,血白细胞计数、血钾、血钠、血肌酐、血淀粉酶、肌酶谱的升高都是反应严重性的重要参考。
[Abstract]:Objective to investigate the relationship between laboratory examination and condition of diabetic ketoacidosis (DKA) and hyperglycemia and hyperosmotic (HHS). Methods 221 patients with diabetic ketoacidosis (including diabetic ketoacidosis complicated with hyperglycemia and hyperosmotic status) admitted to Tongji Hospital from August 01 to 20104 were taken as the subjects. According to the results of arterial blood gas analysis, they were divided into three groups: mild, moderate and severe. The clinical and experimental data of 221 patients with diabetes mellitus admitted to hospital due to acute complications were analyzed retrospectively. Results the results of statistical t test showed that there were significant differences in white blood cell count, serum potassium, serum sodium, serum creatinine, serum amylase, muscle zymogram and severity of disease between DKA and HHS, and there was significant difference between the two groups (P < 0.05), and the correlation between blood leukocyte count, serum potassium, sodium, serum creatinine, serum amylase and muscle zymogram was statistically significant (P < 0.05). There were no significant differences in erythrocyte count, hematocrit, platelet count, blood calcium, magnesium and cholesterol. Conclusion in addition to blood gas analysis is an index to judge the severity of the disease in DKA and HHS, the increase of blood white blood cell count, blood potassium, serum sodium, serum creatinine, serum amylase and muscle zymogram are all important references for the severity of the reaction.
