双相情感障碍患者甲状腺功能水平的分析

【摘要】:目的:探讨双相情感障碍躁狂/轻躁狂发作、抑郁发作与混合发作患者甲状腺功能。方法:采用化学发光免疫分析法测定来自重庆医科大学附属大学城医院心理卫生中心双相情感障碍躁狂/轻躁狂发作患者(62例)、抑郁发作患者(71例)与混合发作患者(26例)以及该院体检中心203名性别、年龄匹配的正常对照(对照组)血清促甲状腺激素(thyroid stimulating hormone,TSH)、游离三碘甲腺原氨酸(free triiodothyronine,FT3)、游离甲状腺素(free thyroxine,FT4)、三碘甲状腺原氨酸(triiodothyronine,T3)及甲状腺素(thyroxine,T4)水平。结果:双相情感障碍躁狂/轻躁狂发作者TSH水平低于[1.650(0.803,2.400)vs.2.100(1.610,3.030),P0.01]、FT4水平高于[18.420(15.530,20.060)vs.16.600(14.890,17.640),P0.01]对照组;抑郁发作者T3、T4水平[1.580(1.180,1.890)vs.1.770(1.610,1.990),84.170(62.540,97.940)vs.95.54(86.020,109.500),P0.01]均低于对照组;混合发作者T3水平[1.440(1.273,1.783)vs.1.770(1.610,1.990),P0.01]低于对照组。与躁狂/轻躁狂发作者相比,抑郁发作者T4、FT4水平均更低[96.39(76.768,123.040)vs.84.170(62.540,97.940),18.420(15.530,20.060)vs.16.520(13.670,19.080),P0.05]。有双相家族史患者血浆中TSH水平高于无家族史双相患者(2.735±2.240 vs.1.892±1.276,P=0.039);女性双相患者T3、T4水平均低于正常女性对照(1.362±0.674 vs.1.769±0.290,78.407±39.180 vs.96.396±22.635,P=0.000)。结论:双相情感障碍抑郁发作、躁狂发作、混合发作时甲状腺功能均有异常,但三者的改变各不相同。
[Abstract]:Objective: to investigate bipolar disorder mania / hypomania. Thyroid function in patients with depression attack and mixed attack. Methods: the bipolar affective disorder patients with mania / hypomania were measured by chemiluminescence immunoassay from the mental health center of the university hospital affiliated to Chongqing medical university. There were 62 patients with depressive attack, 71 patients with depression and 26 patients with mixed attack. Age matched normal control group (control group), the levels of thyroid hormone thyroid stimulating hormonet, free triiodothyronine free triiodothyronine FT3, free thyroxine free thyroxine FT4, triiodothyronine triiodothyronine T3 and thyroxine thyroxine T4). The level of TSH in patients with manic / hypomania attack was lower than that in control group [1.650 卤0.803U 2.400 vs.2.100 + 1.61010 / 3.030U P0.01] FT4 was higher than that in control group [18.420 卤15.530 20.060 vs.16.600 ~ 14.890 卤17.640U P0.01]. The levels of T3OT4 in patients with depression attack [1.580 / 1.180 / 1.890 / vs.1.7701.6 / 1010 / 1.990 / 84.170 / 62.54097.940 / vs.95.5440 / 86.020109.500] were lower than those in the control group, while the T3 level in the patients with mixed attack [1.4401.273n 1.783vs.1.7701.161.61010 / 1.990P0.01] was lower than that in the patients with mania / hypomania (P 0.01), and compared with that in patients with mania / hypomania, the level of T 3 in patients with depression attack was significantly lower than that in patients with hypomania / hypomania (P 0.01). The levels of T4FT-4 in patients with bipolar disorder were significantly lower than those in patients with bipolar family history (96.39) 76.7683.040 / vs.84.170 (62.540 / 97.940) and 18.42015.530 / 20.060vs.16.52013.670 / 19.080V respectively. Conclusion: the level of TSH in patients with bipolar family history is higher than that in patients with bipolar disorder without family history (2.735 卤2.240 vs.1.892 卤1.276P0.039), and the levels of T3T4 in bipolar patients are all lower than those in normal female controls (1.362 卤0.674 vs.1.769 卤0.2978.407 卤39.180 vs.96.396 卤22.635P0. 05). Conclusion: the level of T3 T4 in patients with bipolar family history is 2.735 卤2.240 卤1.276P0.0390.The levels of T3 T4 in patients with bipolar disorder are all lower than those in normal female controls (1.362 卤0.674 vs.1.769 卤0.2978.407 卤39.180 vs.96.396 卤22.635P0. 05). There were abnormal thyroid function in manic attack and mixed attack, but the changes were different among the three groups.
