抑郁症与单纯型精神分裂症鉴别诊断中定量脑电图的应用价值研究

【摘要】:目的探讨抑郁症、单纯型精神分裂症定量脑电图(QEEG)指标的差异,分析QEEG对两种疾病的鉴别诊断价值。方法选取2012年10月—2013年5月无锡市精神卫生中心门诊及收治的住院抑郁症和单纯型精神分裂症患者,各30例,分别作为抑郁症组和单纯型精神分裂症组;另选取同期在无锡市精神卫生中心体检健康者30例(对照组)。三组受试者均行QEEG检测。脑电图频段划分为:δ(0.8~4.0 Hz)、θ(4.0~7.8 Hz)、α(7.8~12.8Hz)、β(13.0~20.0 Hz)。比较三组δ、θ、α、β频段FP1、FP2、F3、F4、F7、F8、T3、T4、T5、T6、C3、C4、P3、P4、O1、O2的功率值。结果三组δ频段FP1、FP2、F3、F4、F7、F8、T3、C3、C4、P3、P4、O1、O2功率值比较,差异均有统计学意义(P0.05),其中单纯型精神分裂症组δ频段FP1、FP2、F3、F4、F7、F8、T3、C3、C4、P3、P4、O1、O2功率值高于对照组,抑郁症组δ频段FP1、FP2、F4、C3、C4、P4、O2高于对照组,单纯型精神分裂症组δ频段FP1、FP2、F3、F7高于抑郁症组(P0.05)。三组θ频段F3、F7、F8、P4、O2功率值比较,差异均有统计学意义(P0.05),其中单纯型精神分裂症组θ频段F3、F7、F8、P4、O2功率值高于对照组,抑郁症组θ频段F8功率值高于对照组(P0.05)。三组α频段FP1、FP2、F3、F4、F7、F8、T3、T4、T5功率值比较,差异均有统计学意义(P0.05),其中单纯型精神分裂症组α频段FP1、FP2、F3、F4、F7、T3、T4、T5功率值低于对照组,抑郁症组α频段FP2、F4、F8功率值低于对照组(P0.05)。三组β频段FP1、FP2、F3、F4、F7、F8、O2功率值比较,差异均有统计学意义(P0.05),其中单纯型精神分裂症组β频段FP1、FP2、F3、F4、O2功率值高于对照组,抑郁症组β频段FP1、FP2、F3、F4、F7、F8功率值高于对照组(P0.05)。结论抑郁症与单纯型精神分裂症患者的QEEG指标有明显差异,QEEG对抑郁症及单纯型精神分裂症的鉴别诊断有重要价值。
[Abstract]:Objective to explore the difference of QEGG in depression and simple schizophrenia, and to analyze the value of QEEG in differential diagnosis of the two diseases. Methods from October 2012 to May 2013, 30 patients with depression and 30 patients with simple schizophrenia were selected as depression group and simple schizophrenia group. Another 30 healthy people in Wuxi Mental Health Center were selected in the same period (control group). QEEG was performed in all three groups. The frequency bands of EEG were as follows: 未 ~ (0.8) ~ (4.0) Hz, 胃 ~ (4. 0) ~ (7.8) Hz, 伪 ~ (7. 8) ~ (12. 8) HzC, 尾 ~ (13. 0) ~ (20.0) Hz ~ (-1). The power values of the three groups of 未, 胃, 伪, 尾 band FP1F2F3F4F4F7F7F8F4T4T5T5T5T5T5T5T5T5T5T5T5T5T5T5T5T6T5T5T5T5T5T5T5T5T5T6C3C4C3C3P4P4P4O@@ Results compared with the control group, there were significant differences among the three groups in the 未 band FP1F2F3F4F7F7F7, T3C3C3C4P3P3P4P4O1O1O 2 power value. In the simple schizophrenia group, the power value of FP1FP2FP2F3F4F7F7F8T3C3C3C3P4P4O1O 2 was higher than that of the control group, and that of the depression group was higher than that of the control group in the 未 frequency band FP1FP2FP2F3F4F7F7F8T3F3F3F4P4O1, and the difference was significant in the patients with depression (P < 0.05), and there was a significant difference between the three groups (P < 0.05), especially in the patients with simple schizophrenia (P < 0.05). The 未 frequency of FP1F2F3F7 in simple schizophrenia group was higher than that in depression group (P0.05). There were significant differences among the three groups (P 0.05) in the power value of F _ 3H _ 7F _ (8) F _ (8) F _ (4) O _ (2) in theta band. The power value of F _ (3) F _ (7) F _ (7) F _ (8) F _ (4) O _ (2) in the pure schizophrenia group was higher than that in the control group, and that in the depression group was higher than that in the control group (P 0.05). There were significant differences among the three groups in the power value of 伪 -band FP1F2F3F3F7F7F7F8T3T4T4T5. The power of 伪 -band FP2F3F3F3T4T5 in simple schizophrenia group was lower than that in the control group, and the power value of 伪 -band FP2F4F4F8 in depression group was lower than that of the control group (P0.05). There were significant differences among the three groups in the power value of FP1FP2F3F4F7F7F8O 2, among which the power value of 尾 -band FP1FP2F3F3F4O 2 in simple schizophrenia group was higher than that of the control group, and the power value of FP1FP2F3F3F4F4F7F8 in depression group was higher than that in the control group (P 0.05). Conclusion there is significant difference in QEEG between patients with depression and simple schizophrenia. QEEG has important value in the differential diagnosis of depression and simple schizophrenia.
