灼口综合征患者心理健康状况及脑功能影像学研究

【摘要】：研究目的： (1)探讨灼口综合征(Burning Mouth Syndrome, BMS)患者的心理健康状况,及各因素之间的相关关系； (2)探讨BMS患者静息态默认网络(default mode network, DMN)的异常改变,从而寻找BMS疾病特异性的生物学标记。研究方法： (1)收集60例BMS患者和60例按性别、年龄和教育程度相匹配的健康对照者,采用抑郁自评量表(Self-rating Depression Scale,SDS)、焦虑自评量表(Self-rating Anxiety Scale, SAS)、医学应对方式量表(Medical Coping Model Questionnaire, MCMQ)和艾森克人格问卷简式量表(Eysenck Personality Questionnaire-Revised, Short Scale for Chinese, EPQRSC)对BMS患者和对照组进行评定和统计分析。 (2)对15例BMS患者和15例年龄、性别、文化程度相匹配的健康对照者进行1.5T静息态功能磁共振扫描,获得13例BMS患者和14例正常对照的有效数据。对数据进行处理后,检测患者组与对照组静息态DMN有显著性差异的脑区。结果： (1)BMS患者的抑郁焦虑症状与其应对方式和人格之间存在显著相关。具体表现为：患者组抑郁、焦虑情绪明显高于对照组；“回避”和“屈服”应对方式与抑郁情绪呈正相关(r回避-抑郁=0.350,r屈服-抑郁=0.684)；精神质、神经质人格与抑郁呈正相关(r精神质-抑郁=0.634,r神经质-抑郁=0.580),与焦虑情绪也呈正相关(r精神质-焦虑=0.550,r神经质-焦虑=0.595)。 (2)BMS患者静息态DMN存在异常。具体表现为：DMN的前部如内侧前额叶、前扣带回功能连接升高,而DMN后部楔前叶、角回功能连接降低(p0.05, t3, False Discovery Rate校正)。结论： (1)BMS患者并发较高程度的抑郁、焦虑症状,同时不同人格类型和应对方式对其焦虑、抑郁有较大影响。因此BMS病人应根据其心理特点加强心理健康治疗。 (2)BMS患者大脑静息态DMN活动异常,特别是内侧前额叶、前扣带回、楔前叶、角回是主要异常部位。该研究提示静息态DMN的异常可能是BMS的神经病理机制之一。
[Abstract]:Objectives of the study: 1) to investigate the mental health status of Burning Mouth Syndrome, BMS) patients with Burning syndrome and the correlation between the factors. (2) to explore the abnormal changes of default mode network, DMN) in resting default network in patients with BMS, and to search for the specific biological markers of BMS disease. Research methods: We collected 60 patients with BMS and 60 healthy controls matched by sex, age and education. Self-rating Depression scale, Self-rating Anxiety scale, Medical coping style scale and Eysenck Personality Questionnaire-Revised, Short Scale for Chinese, EPQRSC) were used to evaluate and analyze BMS patients and controls. (2) Fifteen patients with BMS and 15 healthy controls with matched age, sex and education degree were scanned with 1.5T resting functional magnetic resonance imaging (fMRI). The effective data of 13 BMS patients and 14 normal controls were obtained. After the data were processed, the brain regions with significant difference in resting DMN were detected between the patient group and the control group. Results: There was a significant correlation between depression and anxiety symptoms and coping style and personality in BMS patients. The specific manifestations were as follows: depression and anxiety were significantly higher in the patients group than in the control group, the coping styles of "avoidance" and "yield" were positively correlated with the depression mood, and the coping styles were positively correlated with the depression mood. There was a positive correlation between neuroticism and depression, and a positive correlation between neuroticism and anxiety. There were abnormal resting DMN in patients with BMS. The functional junctions of anterior cingulate gyrus increased in the anterior part of DMN, while the functional junctions of angular gyrus decreased p0.05, T3, False Discovery Rate correction in the posterior cuneate lobe of DMN. Conclusion: BMS patients complicated with high degree of depression, anxiety symptoms, at the same time different personality types and coping styles have a greater impact on their anxiety and depression. Therefore, BMS patients should strengthen mental health treatment according to their psychological characteristics. DMN activity was abnormal in the rest state of brain, especially in medial prefrontal lobe, anterior cingulate gyrus, precuneiform lobe and angular gyrus. This study suggests that the abnormality of resting DMN may be one of the neuropathological mechanisms of BMS.
