网络成瘾青少年脑电图及脑涨落图研究

【摘要】:目的:应用脑电图及脑涨落图技术,分析网络成瘾青少年脑电图和脑内神经递质功率的特点,探讨网络成瘾青少年脑功能的改变。 方法:研究对象分为两组:网络成瘾组和正常对照组。成瘾组38例,为2008年12月至2012年1月期间就诊于湖南省脑科医院成瘾科的成瘾青少年,均符合Young制定的网络成瘾诊断标准,年龄12-29岁,平均年龄20.53±4.45岁。对照组35例,为同期体检结果正常的无网络成瘾青少年,年龄12~29岁,平均年龄20.23±4.32岁。两组研究对象性别、年龄、受教育年限及接触网络年限均相匹配。网络成瘾组的成瘾时间0.5~11年,平均4.46±2.29年。 采用中文网络成瘾量表修订版,对两组受试者进行网络成瘾程度评分。 记录安静、清醒、闭目状态下的脑电图,对所有受试者的图形进行目测阅图和功率值测定。采用SPSS17.0软件对两组受试者目测阅图结果的异常率、异常部位、脑电异常程度和成瘾年限的相关性以及功率值进行统计分析。 通过脑电超慢涨落图技术得到两组受试者的γ-氨基丁酸、谷氨酸、5-羟色胺、乙酰胆碱、去甲肾上腺素、多巴胺六种神经递质的功率值,以及兴奋抑制指数。统计分析两组间六种神经递质功率和兴奋抑制指数,并对差异有显著性的指标与中文网络成瘾量表修订版评分进行相关性分析。 结果:中文网络成瘾量表修订版评分统计分析结果:网络成瘾组与正常对照组比较,量表评分明显增高,差异有统计学意义(P0.05)。 脑电图检查统计分析结果:1)成瘾组与对照组在目测阅图结果的比较:成瘾组目测阅图的异常率较对照组升高,病变部位主要是额区,差异有统计学意义(P0.05);成瘾组的成瘾年限与目测阅图的异常度的相关分析中,P0.05,提示网络成瘾青少年的脑电图异常度与成瘾时间不相关。2)两组脑电功率的比较结果:成瘾组双侧额、颞较正常对照组慢波(δ、0波)的功率增高。 脑涨落图统计分析结果:1)成瘾组与对照组在脑涨落图结果的比较:与对照组相比,成瘾组的多巴胺功率、5-羟色胺功率和兴奋抑制指数降低,差异有统计学意义(P0.05)。2)网络成瘾量表修订版评分与神经递质功率水平的相关性分析结果:网络成瘾量表修订版评分与5-羟色胺功率值、多巴胺功率值以及兴奋抑制指数均呈负相关(P0.05),相关系数分别为-0.323,-0.775,-0.252。 结论:1)脑电图显示网络成瘾青少年额、颞脑区脑功能处于抑制状态,抑制程度与成瘾的时间不相关。脑兴奋性降低是导致网络成瘾的可能原因。 2)脑涨落图显示网络成瘾青少年颅内多巴胺、5-羟色胺功率降低,可能是大脑额、颞叶功能抑制的物质基础。
[Abstract]:Objective: to analyze the characteristics of EEG and the power of neurotransmitters in the brain of the adolescents with internet addiction by using EEG and fluctuation graph, and to explore the changes of brain function in adolescents with iad. Methods: the subjects were divided into two groups: Internet addiction group and normal control group. From December 2008 to January 2012, 38 patients in the addictive group were admitted to the Department of Addiction in the brain Hospital of Hunan Province. All of them were in accordance with the criteria for the diagnosis of Internet addiction established by Young. The average age was 20.53 卤4.45 years old, aged 12-29 years. The control group (n = 35) was a non-internet addicted teenager with normal physical examination results. The average age was 20. 23 卤4. 32 years with a age of 12 to 29 years. Gender, age, years of education and exposure to the Internet were matched equally between the two groups. The addiction time of iad group was 0.5 ~ 11 years (mean 4.46 卤2.29 years). The Chinese Internet Addiction scale was used to evaluate the degree of iad in the two groups. Electroencephalogram (EEG) in quiet, awake and closed state was recorded, and the figures of all subjects were measured by visual reading chart and power value. SPSS17.0 software was used to analyze the abnormal rate, abnormal position, abnormal degree of EEG and addiction years and power value of the two groups of subjects. The power values of 纬 -aminobutyric acid, glutamate 5-hydroxytryptamine, acetylcholine, norepinephrine and dopamine neurotransmitters, as well as the excitatory inhibition index of the two groups were obtained by electroencephalogram. The power of six neurotransmitters and the excitation inhibition index were analyzed statistically and the correlation between the significantly different indexes and the revised score of the Chinese Internet Addiction scale was analyzed. Results: the scores of the revised version of the Chinese Internet Addiction scale were significantly higher in the iad group than in the normal control group (P0.05). The results of EEG statistical analysis showed that the abnormal rate of visual map in addiction group and control group was higher than that in control group, and the lesion location was mainly frontal area, the difference was statistically significant (P0.05). The correlation analysis of addictive years and abnormal degree of visual reading map in addiction group (P0.05, indicating that the abnormal degree of EEG of internet addicted adolescents is not related to addictive time (.2) the results of comparison of EEG power between two groups: the bilateral forehead of addiction group; The power of slow wave (未 0 wave) in temporal group was higher than that in normal control group. The results of brain fluctuation analysis showed that the dopamine power of addiction group was lower than that of control group, and the dopamine power and excitation inhibition index of addiction group were lower than that of control group, compared with the control group, the brain fluctuation analysis showed that the dopamine power and the excitatory inhibition index of the addiction group were lower than those of the control group. The difference was statistically significant (P0.05). The results of correlation analysis between the revised scale of iad and the level of neurotransmitter power were as follows: the revised score of iad and the value of 5-hydroxytryptamine power. There was a negative correlation between dopamine power and excitatory inhibition index (P0.05), the correlation coefficients were -0.323- 0.775U -0.252, respectively. Conclusion (1) EEG showed that the brain function in the brain of iad adolescents was in a state of inhibition, and the degree of inhibition was not related to the time of addiction. The decrease of brain excitability is the possible cause of iad. 2) the decrease of brain dopamine 5-hydroxytryptamine power may be the material basis for the inhibition of brain function in frontal and temporal lobes.
