双相障碍患者情感气质特征的研究

【摘要】：双相障碍是精神科的常见疾病,是一个重要的全球卫生问题,情感气质是人们对环境刺激作出的反应,主要包括个体的生物节律、情绪、认知等[1],国外研究提示一些情感气质是情感障碍的亚临床表现,可能是情感障碍临床表现与生物遗传学联系的纽带[2]。目前国内双相障碍情感气质特征的研究较少,未见此方面的系统研究,本研究分为两部分:第一部分双相障碍情感气质特征以及双相Ⅰ型和Ⅱ型障碍情感气质比较目的:探讨双相障碍患者情感气质的特征,分析双相Ⅰ型障碍和双相Ⅱ型障碍情感气质特征的差异性。方法:采用横断面研究,共纳入160例双相障碍组(双相Ⅰ型组90例、双相Ⅱ型组70例)和对照组150例。采用情感气质评定量表简版中文版评估双相障碍患者及对照组间情感气质类型,比较不同分型情感气质特征的差异性。结果:双相障碍组情感气质评定量表总分及5个因子分均高于对照组,差异有统计学意义(均P0.05),在情感旺盛气质因子得分方面双相Ⅰ型障碍患者高于双相Ⅱ型障碍;双相Ⅰ型障碍患者的抑郁气质因子得分低于双相Ⅱ型障碍,差异均有统计学意义(P0.05)。结论:双相障碍患者存在特殊的情感气质,不同亚型之间情感气质类型有差异;双相Ⅰ型障碍患者具有较高的情感旺盛气质,而双相Ⅱ型障碍患者具有相对较高的抑郁气质特征。第二部分双相障碍患者与健康一级亲属情感气质特征比较目的:探讨双相障碍患者与健康一级亲属情感气质的特征,分析两者之间的差异性。方法:本研究共纳入75例双相障碍患者、健康一级亲属75例和对照组9 0例。采用情感气质评定量表简版中文版评估双相障碍患者及健康一级亲属之间情感气质类型,并对情感气质特征的差异性进行比较。结果:在循环气质因子方面,双相障碍组评分高于健康一级亲属组高于对照组;在抑郁气质因子及焦虑气质因子方面,双相障碍组评分均高于健康一级亲属组和对照组;在易激惹气质因子及情感旺盛气质因子方面,对照组评分分别低于双相障碍组和健康一级亲属组,差异均有统计学意义(均P0.05)。结论:双相障碍患者的健康一级亲属同样存在特殊的情感气质,循环气质最可能是双相障碍“内表型”指标之一。
