MRI乳头体体积测量及其在认知功能障碍性疾病的初步应用

【摘要】:目的; 观察和分析正常成人乳头体(MB)体积的对称性、随年龄演变趋势和性别差异,初步说明MB体积在正F常成年人群的分布情况。观察和分析以轻度认知功能障碍(MCI)和阿尔茨海默病(AD)为代表的认知功能障碍性疾病的MB体积变化,初步探讨认知功能障碍性疾病所致的MB体积变化趋势及其临床应用价值。 方法: 选择正常成人50例和年龄匹配的正常对照、MCI、AD患者各25例。采用GE HD-X3.0T超导型磁共振扫描仪采集正常成人和正常对照MR图像;下载ADNI的MCI、AD患者MR图像。使用ITK-SNAP Version2.0.0软件在三维高分辨率T1WI解剖像测量MB体积,对每个MB重复测量两遍,取平均值为测量体积,取同一研究对象双侧测量体积之和为测量总体积,并以全脑体积进行标准化处理得到标化体积和标化总体积。采用SPSS19.0统计软件包对MB标化体积指标进行统计分析:正常成人侧别、年龄段、性别分别进行配对样本,检验、单因素方差分析和独立样本t检验;MCI、AD组患者侧别、性别分别进行配对样本,检验、独立样本t检验;正常对照、MCI、AD组间进行单因素方差分析。 结果: 正常成人MB标化体积和标化总体积平均值为100.2±11.2mm3和200.4±20.1mm3,按照侧别、年龄段和性别分析MB标化体积指标均无统计学差异。MCI、AD组内侧别研究显示,MB标化体积右侧(MCI92.2±19.4mm3, AD79.1±14.5mm3)大于左侧(MCI82.9±17.2mm3, AD71.2±15.6mm3),差异均存在统计学意义(tmC1=3.920,tAD=3.422,p0.05)。MCI、AD组内性别比较显示,两性间MB标化体积指标无统计学差异。正常对照,MCI及AD组间MB标化体积、标化总体积单因素方差分析均存在统计学差异(F标化体积=35.965,F标化总体积=21.610,p0.05),并且正常对照(标化体积101.3±11.0mm3、标化总体积202.5±19.7mm3)MCI(标化体积87.6±18.8mm3、标化总体积175.2±34.7mm3)AD(标化体积75.2±15.4mm3、标化总体积1503±27.8mm3),差异均存在统计学意义(p0.05);三组间右侧、左侧组MB标化体积单因素方差分析同样存在统计学差异(F右侧标化体积=14.782,F左侧标化体积=23.366,p0.05),并且正常对照(右侧102.5±10.4mm3、左侧100.0±11.7mm3)MCI(右侧92.2±19.41mm3、左侧82.9±17.2mm3)AD(右侧79.1±14.5mm3、左侧71.2±15.6mm3),差异均存在统计学意义(p0.05)。 结论: 1.正常成人MB的体积分布特点:双侧基本对称、无明显性别差异,坡重要的是随年龄增加萎缩不明显。 2.认知功能障碍性疾病,如MCI、AD可伴有MB体积萎缩;MB体积萎缩可于AD早期出现,且与病变进程相一致,可作为AD早期较为敏感的脑体积变化指标,但特异性差 3.
